机械通气联合膨肺吸痰法对矽肺合并呼吸衰竭患者疗效及护理研究

【摘要】：目的探讨机械通气联合膨肺吸痰法治疗矽肺合并呼吸衰竭患者的疗效及护理。方法以2013—2016年治疗的76例矽肺合并呼吸衰竭患者为研究对象。采用随机数字表法分为对照组38例和观察组38例,对照组进行常规吸痰和人工气道护理;观察组在对照组基础上进行膨肺吸痰护理。治疗1周后,比较2组患者气道阻力、血氧分压(PaO_2)、血氧饱和度(SaO_2)、呼吸机相关性肺炎(VAP)发生率、病死率、肺不张发生率的情况。结果治疗前2组患者气道阻力、PaO_2和SaO_2分别比较,差异均无统计学意义(P0.05)。治疗后2组患者分别与治疗前同组患者比较,气道阻力均下降(P0.01),PaO_2和SaO_2均升高(P0.01)。治疗后,观察组患者气道阻力低于对照组(P0.01),PaO_2和SaO_2均高于对照组(P0.01)。与对照组比较,观察组患者VAP发生率、病死率和肺不张发生率均低于对照组(P0.05)。结论机械通气联合膨肺吸痰法治疗矽肺合并呼吸衰竭,有利于降低VAP的发生率和病死率,改善患者预后。
[Abstract]:Objective to investigate the curative effect of mechanical ventilation combined with lung dilated suction phlegm treatment and nursing of patients with silicosis complicated with respiratory failure. Methods 2013 2016 treatment of 76 cases of silicosis complicated with respiratory failure patients as the research object. They were randomly divided into control group of 38 cases and 38 cases in the observation group, the control group was given routine sputum aspiration and artificial airway nursing; the observation group in the control group on the basis of lung suctioning care. After 1 weeks of treatment, compared 2 groups of patients with airway resistance, oxygen partial pressure (PaO_2), oxygen saturation (SaO_2), ventilator-associated pneumonia (VAP) incidence rate, mortality rate, incidence rate of pulmonary atelectasis. The treatment results the 2 groups of patients with airway resistance, PaO_2 and SaO_2 respectively, there were no significant differences (P0.05). After treatment, 2 groups of patients before and after treatment respectively with the group of patients, airway resistance decreased (P0.01), PaO_2 and SaO_2 were higher (P0.01). After treatment, the observation group patients Their airway resistance is lower than the control group (P0.01), PaO_2 and SaO_2 were higher than the control group (P0.01). Compared with the control group, the observation group of patients with VAP incidence, mortality and the incidence of atelectasis were lower than the control group (P0.05). Conclusion mechanical ventilation combined with lung dilatedsuction treatment of silicosis complicated with respiratory failure. To reduce the incidence of VAP and mortality, improve the prognosis of patients.
