急诊科护士情绪管理及临床沟通能力的调查研究

【摘要】:目的调查急诊科护士情绪管理及临床沟通能力现状,分析影响其情绪管理的因素,探讨急诊科护士情绪管理与临床沟通能力之间的相关性,为护理管理者开展临床护士情绪管理培训、提升临床护士沟通能力提供相关依据。方法选取2013年10~12月青岛市三所三甲医院182名急诊科护士进行问卷调查。调查工具包括急诊科护士的一般资料调查表、护士情绪管理量表、护士临床沟通能力量表三部分。利用SPSS l7.0进行数据统计分析,统计方法包括描述性分析、单因素分析、Spearman相关性分析及多元逐步回归分析。结果1、急诊科护士情绪管理现状共发放调查问卷200份,收回有效问卷182份,有效回收率91%;急诊科护士情绪管理处在中等水平以上,情绪管理总分(49.59±4.83)分,得分率为69%;四个维度按照其得分率的高低排序依次是情绪调整(14.20±1.93)分、情绪觉察(14.03±1.52)分、情绪运用(13.83±1.82)分、情绪表达(7.52±1.23)分;2、急诊科护士临床沟通能力现状调查示急诊科护士临床沟通能力处于中等以上水平,临床沟通能力总分为(212.40±28.14)分,各维度得分由高到低分别为困难情景沟通能力(72.93±10.23)分、团队沟通能力(22.65±3.35)分、基本语言沟通能力(40.40±5.81)分、基本非语言沟通能力(25.60±3.84)分、情感支持能力(21.65±3.52)分、情感感知能力(29.16±4.81)分。3、急诊科护士情绪管理影响因素分析单因素分析显示不同性别、年龄、学历、婚姻状况、聘用形式对急诊科护士情绪管理得分差异无统计学意义(P㧐0.05);不同护龄、职称、是否遭遇过工作中的暴力行为、工作倦怠及情绪转移对急诊科护士情绪管理得分差异有统计学意义(P㩳0.05);以急诊科护士情绪管理为因变量,护龄、职称、是否遭遇过工作中的暴力行为、工作倦怠、情绪转移以及临床沟通能力进行多元逐步回归分析,最终工作倦怠、情绪转移与临床沟通能力三个变量进入回归方程,工作倦怠与情绪转移回归系数均为负值,说明进入方程的两个变量对急诊科护士情绪管理有负向预测作用;临床沟通能力对急诊科护士情绪管理有正向预测性;经检验回归方程F为14.542,具有统计学意义(P㩳0.05)。4、急诊科护士情绪管理与临床沟通能力的相关性分析急诊科护士情绪管理与临床沟通能力呈正相关,其中情绪调整维度、情绪运用维度及情绪管理总分分别与临床沟通能力各维度呈显著正相关(P㩳0.01);而情绪表达维度相关系数最大为r=0.579,情绪觉察维度相关系数最大r=0.230。结论急诊科护士情绪管理处于中等以上水平,建议应将情绪管理引入护理管理领域,本着以人为本的管理理念做好情绪管理,进一步提高临床护理的服务与质量。
[Abstract]:Objective to investigate the present situation of emotional management and clinical communication ability of nurses in emergency department, analyze the factors affecting their emotional management, and explore the correlation between emotional management and clinical communication ability of nurses in emergency department. To carry out emotional management training for clinical nurses for nursing managers, Methods 182 nurses in emergency department of three grade A hospitals in Qingdao city were investigated by questionnaire from October to December 2013. The survey tools included general information questionnaire of nurses in emergency department and nurses' emotional management scale. Three parts of nurses' clinical communication ability scale. Using SPSS L7.0 to carry on the statistical analysis, the statistical method includes descriptive analysis, Single factor analysis, Spearman correlation analysis and multiple stepwise regression analysis. Results 1. A total of 200 questionnaires were distributed and 182 valid questionnaires were collected. The effective recovery rate was 91%, the total score of emotion management was 49.59 卤4.83) and the score rate was 690.The order of the four dimensions was emotion adjustment 14.20 卤1.93), emotion perception 14.03 卤1.52), emotion application 13.83 卤1.82). The present situation of clinical communication ability of nurses in emergency department was found to be above the average level, and the total score of clinical communication ability was 212.40 卤28.14. The scores from high to low were 72.93 卤10.23, 22.65 卤3.35, 40.40 卤5.81, 25.60 卤3.84 and 21.65 卤3.52, respectively. Emotion perception ability was 29.16 卤4.81) score. 3. Single factor analysis showed that there was no significant difference between gender, age, educational background, marital status, employment form and emotional management score of emergency department nurses. 0. 05%; different nursing years and professional titles, have you ever encountered violence at work, job burnout and emotional transfer have significant differences in emotional management scores of nurses in the emergency department (P < 0. 05)? Using emotional management of emergency department nurses as dependent variables, nursing age, professional title, whether or not they have experienced violence in work, job burnout, emotional transfer and clinical communication ability were analyzed by multivariate stepwise regression analysis, and finally job burnout was obtained. Three variables of emotional transfer and clinical communication ability entered the regression equation, and the regression coefficients of job burnout and emotional transfer were all negative, indicating that the two variables entered into the equation had a negative predictive effect on the emotional management of the nurses in the emergency department. The clinical communication ability had positive predictive effect on the emotional management of the nurses in the emergency department, and the regression equation F was 14.542, which had statistical significance. Correlation between emotional Management and Clinical Communication ability of Nurses in Emergency Department; emotional Management and Clinical Communication ability of Nurses in Emergency Department were positively correlated, among which emotional adjustment dimension, Emotional use dimension and total score of emotion management were significantly positively correlated with each dimension of clinical communication ability (P < 0.05). The correlation coefficient of emotion expression dimension and emotion awareness dimension was the highest, r = 0.579 and r = 0.230.Conclusion the emotional management of nurses in emergency department is above the middle level, and it is suggested that the emotional management should be introduced into the field of nursing management. In line with the people-oriented management concept to do a good job in emotional management, and further improve the clinical nursing service and quality.
