口咽训练治疗阻塞型睡眠呼吸暂停综合征的Meta分析

【摘要】:目的采用Meta分析综合评价口咽训练对阻塞型睡眠呼吸暂停综合征(OSAS)的作用。方法利用计算机检索国内外发表的有关口咽训练治疗OSAS的文献,按纳入和排除标准,经质量评价后确定最终文献,提取有关指标数据,应用RevMan 5.3软件进行分析。结果共纳入6项报道了多导睡眠图和嗜睡情况的研究,包括92例研究对象。Meta分析结果显示,口咽训练后患者的呼吸暂停低通气指数(AHI)明显低于训练前,合并后均数差(MD)=14.26,95%CI(7.54,20.98),Z=4.16(P0.01)。夜间最低血氧饱和度(SaO2)在训练后有显著提高,合并后MD=-3.66,95%CI(-6.06,-1.26),Z=2.99(P0.01)。Epworth嗜睡量表评分结果显示,口咽训练后评分下降,合并后MD=5.27,95%CI(1.97,8.57),Z=3.13(P=0.002)。3个Meta分析均存在异质性,故采用随机效应模型分析。结论口咽训练可减低OSAS患者的AHI指数,提高夜间SaO2和改善嗜睡等症状,可为患者的康复提供一种新的无创性治疗方法。
[Abstract]:Objective to evaluate the effect of oropharynx training on obstructive sleep apnea syndrome (OSAS) by Meta analysis. After the quality evaluation, the final literature was determined, the relevant index data were extracted and analyzed by RevMan 5.3 software. The results included 6 studies on polysomnography and somnolence, including 92 cases of subjects. Meta analysis showed that, The apnea hypopnea index (AHII) of the patients after oropharyngeal training was significantly lower than that before training, and the mean difference after the combined exercise was 14.2695CII 7.54 ~ 20.98 ~ 4.16p 0.01g. The lowest nocturnal oxygen saturation (SaO2) increased significantly after training. The MD-3.695CI-6.06CI-6.06- 1.2606-2.99g P0.01. Epworth sleepiness scale score showed that. After oropharynx training, the score decreased, and after the combination, the score of MDN 5.27995 CIQ 1.97 / 8.57z3.13P0. 0020.The 3 Meta analysis showed heterogeneity, so the random effect model was used. Conclusion oropharynx training can reduce the AHI index of OSAS patients, increase the nocturnal SaO2 and improve the symptoms of lethargy, etc. It can provide a new non-invasive therapy for the rehabilitation of patients.
