ICU耐碳青霉烯类鲍曼不动杆菌医院感染暴发调查

【摘要】:目的调查某院重症监护科发生的耐碳青霉烯鲍曼不动杆菌医院感染暴发的原因。方法采取回顾病程记录、实验室检查和前瞻性调查的方法,对2015年8月6日~27日该院重症监护科发生的5例下呼吸道耐碳青霉烯鲍曼不动杆菌感染进行流行病学调查,对疑似污染环境和物品进行微生物采样,对细菌培养结果进行微生物鉴定和药敏试验。结果 5例患者痰培养分离出耐碳青霉烯鲍曼不动杆菌,除对替加环素敏感和左氧氟沙星中介外,其他均耐药。5例患者2例经治疗好转出院,3例死亡。共采集环境物表标本84份,其中从监护导联线和保洁工人手分离到耐碳青霉烯鲍曼不动杆菌。经采取感染防控综合措施后,控制了疫情的蔓延。结论此次重症监护科耐碳青霉烯鲍曼不动杆菌医院感染暴发可能与保洁工人手和/或监护导联线污染相关。
[Abstract]:Objective to investigate the causes of nosocomial infection of Acinetobacter baumannii in a hospital. An epidemiological investigation was carried out on 5 cases of Acinetobacter baumannii infection in lower respiratory tract from 6 to 27 August 2015 in the Department of intensive Care in this hospital. The suspected contaminated environment and articles were sampled by microorganism. Results Acinetobacter baumannii was isolated from sputum culture of 5 patients, except for tegacycline and levofloxacin. Two patients with other drug resistance were cured and 3 died. A total of 84 environmental surface specimens were collected, among which Acinetobacter Carbicilli was isolated from the lead wire and the hands of cleaning workers. After the comprehensive measures of infection prevention and control were taken, Conclusion the nosocomial infection outbreak of Acinetobacter baumannii in intensive care department may be related to hand and / or lead line contamination of cleaning workers.
