河南地区耐甲氧西林金黄色葡萄球菌SCCmec分型与耐药性分析

【摘要】:目的检测临床分离的耐甲氧西林金黄色葡萄球菌(MRSA)葡萄球菌染色体mec盒(SCCmec)基因型并根据分型结果分析耐药性,为临床治疗MRSA感染提供参考依据。方法收集2013年8月-2014年5月临床分离的金黄色葡萄球菌122株,采用VITEK-2全自动微生物分析仪完成药敏鉴定,普通PCR技术检测mecA基因,并采用多重PCR对其进行SCCmec基因型分型。结果 122株金黄色葡萄球菌,经mecA基因筛选后,检出MRSA 47株,检出率38.5%,SCCmec分型结果表明,SCCmecⅢ型菌株35株占74.5%,SCCmecⅡ型菌株6株占12.8%,SCCmecV型2株占4.3%,未分型菌株4株占8.5%;SCCmecⅡ型、SCCmecⅢ型除了对β-内酰胺类药物广泛耐药外,还对喹诺酮类、大环内酯类、克林霉素、四环素具有较高的抗药活性,而SCCmecV型仅对红霉素、克林霉素、四环素耐药;所有菌株均对呋喃妥因、万古霉素、替加环素、利奈唑胺稳定敏感。结论医院临床分离的MRSA以SCCmecⅢ型为主,其次为SCCmecⅡ型,并有少量SCCmecV型出现;SCCmecⅢ型和SCCmecⅡ型对抗菌药物均呈多药耐药性。
[Abstract]:Objective to detect the genotypes of methicillin-resistant Staphylococcus aureus (MRSA) chromosome mec cassette (SCC mecc) and to analyze the drug resistance according to the results of the genotyping so as to provide a reference for clinical treatment of MRSA infection. Methods 122 strains of Staphylococcus aureus isolated from August 2013 to May 2014 were collected. The drug sensitivity was identified by VITEK-2 automatic microbiological analyzer. The mecA gene was detected by ordinary PCR technique. The SCCmec genotypes were genotyped by multiple PCR. Results among the 122 strains of Staphylococcus aureus, 47 strains of MRSA were detected by mecA gene screening. The results of SCCmec typing showed that 35 strains of SCCmec 鈪,
