泛耐药铜绿假单胞菌β-内酰胺类与氨基糖苷类耐药元件研究

【摘要】:目的探讨泛耐药铜绿假单胞菌β-内酰胺类与氨基糖苷类耐药基因与可移动遗传元件的携带及菌株间的亲缘关系,为临床治疗提供参考依据。方法收集2014年1-12月连云港市第二人民医院患者痰液标本分离出的22株泛耐药铜绿假单胞菌,采用聚合酶链反应(PCR)及序列分析的方法分析12种β-内酰胺类耐药基因、6种氨基糖苷类耐药基因、3种可移动遗传元件遗传标记,并对检测结果作样本聚类分析,数据采用SPSS 17.0软件进行统计分析。结果 22株泛耐药铜绿假单胞菌共检出6种β-内酰胺类耐药基因、5种氨基糖苷类耐药基因、3种可移动遗传元件遗传标记,且阳性率较高;样本聚类分析提示22株泛耐药铜绿假单胞菌分为A1、A2、B的3个家族,这3个家族均提示为医院感染。结论泛耐药铜绿假单胞菌中携带的耐药基因和耐药表型相对应,携带blaIMP、blaTEM、blaCARB、blaOXA-10群、blaVEB基因和伴膜孔蛋白基因oprD2缺失以及携带若干个氨基糖苷类耐药基因,是泛耐药铜绿假单胞菌对β-内酰胺类和氨基糖苷类耐药的重要原因。
[Abstract]:Objective to investigate the relationship between 尾 -lactam and aminoglycoside resistant genes and transportable genetic elements in pan-resistant Pseudomonas aeruginosa and to provide reference for clinical treatment. Methods A total of 22 pan-resistant Pseudomonas aeruginosa strains isolated from sputum of patients in Lianyungang second people's Hospital from January to December 2014 were collected. Polymerase chain reaction (PCR) and sequence analysis were used to analyze 12 尾 -lactam resistance genes, 6 aminoglycoside resistance genes and 3 transportable genetic element genetic markers. The data were analyzed by SPSS 17.0 software. Results A total of 6 尾 -lactam resistant genes, 5 aminoglycoside resistance genes and 3 transportable genetic elements were identified in 22 pan-resistant Pseudomonas aeruginosa strains, and the positive rate was high. Cluster analysis showed that 22 strains of pan-resistant Pseudomonas aeruginosa were classified into three families of A1A2B, all of which were identified as nosocomial infection. Conclusion the drug-resistant genes contained in pan-resistant Pseudomonas aeruginosa correspond to the phenotypes of drug resistance. They carry the oprD2 deletion of blaVEB gene and membrane porin gene and several aminoglycoside resistance genes. Pan-resistant Pseudomonas aeruginosa is an important cause of 尾-lactam and aminoglycoside resistance.
