20株耐药阴沟肠杆菌可移动耐药元件检测及样本聚类分析

【摘要】:目的调查一组耐药阴沟肠杆菌中可移动耐药元件(可移动遗传元件遗传标记与耐药基因)的携带情况,以及本组菌株的亲缘关系。方法收集医院2015年1至12月住院病人痰液标本中分离的耐药阴沟肠杆菌共20株,采用聚合酶链反应(PCR)及序列分析的方法分析11种可移动遗传元件与20种β-内酰胺类耐药基因、16种氨基糖苷类耐药基因。并对结果进行了样本聚类分析。结果有19株阴沟肠杆菌检出可移动遗传元件遗传标记、β-内酰胺类和氨基糖苷类耐药基因。20株耐药阴沟肠杆菌共检出5种可移动遗传元件遗传标记:tnp513、tnpU、IS26、IS903、intⅠ1,4种β-内酰胺类耐药基因:blaTEM、blaOXA-1群、blaDHA、blaACT/MIR群,6种氨基糖苷类耐药基因:aac(3)-Ⅱ、aac(6′)-Ⅰb、ant(3″)-Ⅰ、ant(2″)-Ⅰ、aph3′-Ⅵ、rmtB。样本聚类分析提示,除2号菌与18号菌外,其余18株显聚集性。3-9-10-16号菌株,1-17号菌株,5-14号菌株,15-19号菌株均为克隆传播。结论本组耐药阴沟肠杆菌检出的可移动耐药元件是导致对β-内酰胺类、氨基糖苷类耐药的重要原因,并且耐药表型和基因型一一对应。本组菌中4个不同的克隆传播,有医院感染的可能。
[Abstract]:Objective to investigate the carrying status of a group of drug-resistant Enterobacter cloacae (Drug-resistant strains) and the relationship between the transportable elements (transportable genetic elements genetic markers and drug-resistant genes). Methods A total of 20 strains of drug-resistant Enterobacter cloacae isolated from sputum samples of hospitalized patients from January to December 2015 were collected. Polymerase chain reaction (PCR) and sequence analysis were used to analyze 11 transportable genetic elements and 20 尾 -lactam resistant genes and 16 aminoglycoside resistant genes. The results are analyzed by sample clustering. Results A total of 19 strains of Enterobacter cloacae were found to have transportable genetic element genetic markers, and 尾 -lactam and aminoglycoside resistance genes. 20 strains of drug-resistant Enterobacter cloacae were found to have 5 transportable genetic element genetic markers: tnp513TnpUIS26TnpUIS26S903int 鈪,
