VITEK MS基质辅助激光解析电离飞行时间质谱在病原菌鉴定中的应用评价

【摘要】:目的使用VITEK MS质谱鉴定系统对临床分离的病原菌进行鉴定,并评价其性能,为病原菌的快速鉴定提供新的方法。方法收集2014年4-7月医院临床分离病原菌共948株,经VITEK-2Compact细菌鉴定仪及16SrRNA、rpoB、hisA、ITS基因片段测序确认,使用VITEK MS基质辅助激光解析电离飞行时间质谱进行鉴定,并对8株真菌连续检测测试质谱仪稳定性。结果共分离到链球菌属191株、肠球菌属94株、葡萄球菌属144株、肠杆菌科细菌193株、非发酵革兰阴性菌170株、临床常见其他菌36株、酵母菌属102株和丝状真菌18株。VITEK MS对入选菌株的鉴定总准确率达97.10%,但其对沙门菌属和鲍氏不动复合群的鉴定仅能到菌属或复合群水平;对酵母菌属的准确鉴定率为100.00%,丝状真菌中除链格孢外(数据库未包含此菌),88.89%的丝状真菌得到准确鉴定;VITEK MS质谱仪对真菌连续5d检测结果一致,稳定性良好。结论 VITKE MS表现稳定、准确率高、耗时短,尤其在真菌的鉴定上凸显优势,可作为临床细菌快速鉴定的手段。
[Abstract]:Objective to identify and evaluate the clinical pathogenic bacteria by VITEK MS mass spectrometry, and to provide a new method for rapid identification of pathogenic bacteria. Methods A total of 948 strains of pathogenic bacteria were collected from hospitals from April to July 2014. The results were confirmed by VITEK-2Compact bacteria identification instrument and 16s rRNA-rpoBnb hisAITS gene fragment sequencing. VITEK MS matrix-assisted laser desorption ionization time of flight mass spectrometry was used to identify 948 strains of pathogenic bacteria. The stability of mass spectrometer was tested by continuous detection of 8 strains of fungi. Results A total of 191 strains of Streptococcus, 94 strains of Enterococcus, 144 strains of Staphylococcus, 193 strains of Enterobacteriaceae, 170 strains of non-fermentative Gram-negative bacteria, 36 strains of common clinical bacteria were isolated. 102 strains of yeast and 18 filamentous fungi. VITEK MS had a total accuracy of 97.1010, but the identification of Salmonella and Baumer's immobility complex could only reach the level of genus or complex. The accurate identification rate of the genus yeast was 100.00g, and 88.89% of filamentous fungi except Alternaria (which was not included in the database) were identified accurately by vitek MS mass spectrometer for 5 consecutive days, and the results were consistent, and the stability of the filamentous fungi was good. Conclusion VITKE MS has the advantages of stable performance, high accuracy, short time consuming, especially in the identification of fungi. It can be used as a method for rapid identification of clinical bacteria.
