冰冻解冻去甘油红细胞使用MAP悬浮液对红细胞保存期的影响

【摘要】:目的评价MAP(枸橼酸三钠-枸橼酸-葡萄糖-磷酸二氢钠-氯化钠-腺嘌呤-甘露醇)作为冰冻解冻去甘油红细胞悬浮保养液对血液生化指标的影响,参照《全血及成分血质量要求》,探讨使用MAP作为悬浮液的冰冻解冻去甘油红细胞保存期。方法实验用60袋血液均来自苏州市无偿献血人群,由400ml全血制备的2u(单位)悬浮红细胞。于采血后6天内,2-6℃保存,制备冰冻红细胞。解冻后的红细胞平均分为2袋,分别用MAP和NS(0.9%氯化钠注射液)悬浮保存,故本研究分为MAP组(n=60)和NS组(n=60)。M AP组以MAP和红细胞按1:2的比例加入MAP液悬浮保存,放置在4℃冰箱储存,分别于0d、1d、7d、14d、21d、28d、35d从每袋血液无菌操作取样,测定并观察Hb、F Hb、HCT、K+、Na+、溶血率质量指标的变化。NS组为对照组分别于0d、1d、7d无菌操作取样,测定并观察Hb、FHb、HCT、K+、Na+、溶血率质量指标的变化。结果随着保存时间的延长,MAP组实验数据发现FHb在1d、7d时远远低于NS组的数值,可以说明MAP组在1d、7d时被破坏的红细胞数量少于NS组,并且在1d时NS组就有3/60样本就超过了成分血质量标准的数值。Hb在1d、7d时MAP组均高于NS组的数值,HCT在1d、7d时MAP组略高于对照组的数值,溶血率在1d、7d时MAP组低于NS组的数值,均可以证明MAP组在1d、7d时红细胞总数量高于NS组。K+在1d、7d时实验组低于对照组的数值,也可以说明MAP组的红细胞破损、老化的较NS组少。甘油残留量均符合要求,无菌实验为阴性。结论MAP液用于悬浮保存冰冻解冻红细胞,有较好的保护作用,优于目前使用的NS液。根据《全血及成分血质量要求》,使用MAP液悬浮保存冰冻解冻红细胞,各项质量指标的实验数据在7d内均符合要求。
