构建“H2H”营养管理模式——以肿瘤患者为例

【摘要】：本文介绍了"H2H"(Hospital to Home)营养管理模式,是一种由四川大学华西医院临床营养科在国内首次提出的连续的、个体化的营养管理模式,是把患者的营养治疗从医院扩展到出院/院外,将单一的治疗方式丰富为多形式的治疗方案,以患者为中心,参与人员不仅包括临床营养师、专科医生、社区医生和护士,患者家属也应积极参与进来,减少再入院的可能。本文结合国内外现状,针对可能存在的问题提出对策,并以肿瘤患者为例,解读如何建立该模式,以达到提高患者依从性,减少再入院的目的,为"H2H"营养管理模式的推广应用提供决策支持和科学依据。
[Abstract]:This paper introduces the "H2H" (Hospital to Home) nutrition management model, which is a continuous and individualized nutrition management model proposed by the Clinical Nutrition Department of West China Hospital of Sichuan University for the first time in China. It is to expand the nutritional treatment of patients from hospital to discharge / out of hospital. In order to reduce the possibility of readmission, the single treatment method should be enriched as a multi-form treatment plan, with patients as the center, the participants not only include the clinical dieticians, specialists, community doctors and nurses, but also the family members of the patients. According to the present situation at home and abroad, this paper puts forward the countermeasures against the possible problems, and taking cancer patients as an example, explains how to establish this model in order to improve patients' compliance and reduce readmission. To provide decision support and scientific basis for the popularization and application of H 2 H nutrition management model.
