新医改下公立医院医师职业压力与影响因素实证研究

【摘要】:在当前经济高速发展、社会结构快速转型的背景下,医疗体制改革不断深入,医师已经成为三高(高技术、高风险、高压力)人群。随着生活节奏的加快,医师群体不仅在工作中承担着较重的工作负荷,在生活和学习中也越来越担负着过重的压力。深入探究医师职业压力及产生压力的影响因素制定缓解医师职业压力的措施并实施干预,不仅对缓解医师职业压力、提高医疗质量、缓解医患矛盾具有重要意义,同时为国家制定更加完善的医疗卫生政策进一步提供科学依据。本文在查阅国内外文献及相关文件的基础上,在参考国外已有的职业压力量表基础基础上,结合我省目前医师群体职业压力现状,在此基础上采用自编式问卷,重点对安徽省几所公立三甲医院的226名医师工作者进行个案研究,利用SPSS和EpiData软件进行统计与分析并得出结论。归纳总结医师群体所面对的职业压力状况,具体情况主要表现为:职业压力在年龄、职称、工作年限性等方面存在明显差异;并对职业压力产生的影响因素加以分析:工作任务负荷过重、科研压力重、医疗风险突出、医患矛盾严峻、收入不满意、新医改政策不到位等是主要影响医师职业压力产生的因素。本文最后从政府与社会、组织、个人三个方面提出缓解医院医师职业压力的对策建议。包括:切实履行政府职能,深化医疗卫生体制改革;改革医疗保障制度,推进大病保险;营造宽松的社会舆论环境;医院内部组织管理方面要加强科学共同体内部约束;解决医疗服务与科研工作的冲突,创设宽松的医疗与科研内部环境;个人要正视职业压力,积极主动地去调节。正文共分为四个部分,第一部分主要介绍了相关背景及研究的目的与意义。第二部分主要是对医师职业压力的一个概述。第三部分以安徽省几家公立三甲医院医师的职业压力状况为个案,通过实证研究得出新医改下医师的职业压力状况并分析其产生职业压力的因素。第四部分是在第三章结论的基础之上,提出缓解医师职业压力的对策建议。
[Abstract]:With the rapid development of economy and the rapid transformation of social structure, the reform of the medical system is deepening, and doctors have become the "three high" (high technology, high risk, high pressure) people. With the acceleration of life rhythm, doctors not only bear a heavy workload in their work, but also bear more and more heavy pressure in their life and study. It is of great significance to explore the influencing factors of doctors' occupational stress and to make measures to relieve doctors' occupational pressure and to carry out intervention, which is of great significance not only to relieve doctors' occupational pressure, improve the quality of medical treatment and alleviate the contradiction between doctors and patients, but also to improve the quality of medical treatment. At the same time for the country to formulate more perfect medical and health policy to provide further scientific basis. On the basis of consulting domestic and foreign literature and related documents, and referring to the existing occupational stress scale in foreign countries, combined with the current situation of occupational stress among doctors in our province, this paper adopts a self-made questionnaire. This paper focuses on the case study of 226 physician workers in several public third Class A hospitals in Anhui Province, and makes statistics and analysis by using SPSS and EpiData software and draws a conclusion. Summing up the occupational stress status faced by the group of doctors, the specific situation is as follows: there are obvious differences in age, professional title, working life and so on; The influencing factors of occupational stress are analyzed as follows: heavy workload, heavy scientific research pressure, prominent medical risk, severe contradiction between doctors and patients, unsatisfactory income. The new medical reform policy is not in place and so on are the main factors that affect the professional pressure of doctors. Finally, this paper puts forward countermeasures and suggestions to relieve the occupational pressure of hospital doctors from three aspects: government, society, organization and individual. It includes: carrying out the functions of government, deepening the reform of medical and health system, reforming the medical security system, promoting the serious illness insurance, creating a loose social public opinion environment, strengthening the internal restriction of the scientific community in the internal organization and management of the hospital; To solve the conflict between medical service and scientific research, to create a loose internal environment of medical treatment and scientific research, individuals should face up to the occupational pressure and actively regulate it. The text is divided into four parts. The first part mainly introduces the background and the purpose and significance of the research. The second part is an overview of physician occupational stress. The third part takes the occupational stress status of doctors in several public third Class A hospitals in Anhui Province as a case study through empirical research to get the occupational stress status of doctors under the new medical reform and analyze the factors that lead to occupational stress. The fourth part is based on the conclusion of the third chapter, put forward countermeasures and suggestions to relieve the occupational stress.
