幽门螺杆菌感染管理指南的系统评价

【摘要】：目的循证评价幽门螺杆菌(H.pylori)感染管理指南质量,以促进高质量指南的临床应用,并推动低质量指南的持续改进。方法计算机检索PubMed、CNKI和GIN(Guidelines International Network)、NGC(National Guideline Clearinghouse)、NICE(National Institute for Health and Clinical Excellence)、SIGN(Scottish Intercollegiate Guidelines Network)、ASHP(American Society of Health-System Pharmacists)网站及Google搜索引擎,搜集近10年H.pylori感染管理的相关指南,检索时限均为2005年1月至2015年4月。采用AGREEⅡ工具评价指南的方法学质量,比较不同指南在H.pylori根除指征及推荐的一线根除疗法上的不同。结果共纳入13个指南。在6个评价领域中,清晰性领域表现最佳,平均得分为92.6%(78%~100%);编撰的独立性领域表现最差,平均得分为35.7%(0~92%);范围与目的、指南制定参与人员、制定严谨性、应用性领域表现一般,平均得分分别为70.2%(39%~100%)、41.4%(22%~75%)、41.7%(11%~82%)、58.1%(35%~85%)。结论 H.pylori感染管理指南总体质量不高。有待开发可信赖的、高质量的H.pylori感染管理指南,尤其在编撰独立性、指南制定参与人员及制定严谨性三个方面需要进一步提高。
