医疗物联网水平评价模型研究

【摘要】:[研究背景]当前,物联网作为新一代信息技术广泛应用于各大领域,在医疗方面,物联网技术具有优化医疗服务模式、提高医院营运和管理效率,提高医院整体服务水平等多种优势,虽然我国医疗物联网建设正快速开展,但目前还没有关于医疗物联网水平评价的指标体系和具体应用,医疗物联网建设是一套复杂的系统工程,具体发展水平及建设效益很难从主观上把握,因此,迫切需要一套科学权威的评价指标体系。[研究目的]在分析医疗物联网水平评价现状及存在问题的基础上,构建一套科学权威的、具有较高信度效度的医疗物联网水平评价体系,来促进医疗物联网建设的规范化和科学化。[研究方法和研究内容]①本研究通过文献分析法,查阅国内外医疗物联网评价相关文献,为评价指标体系提供理论依据,并结合近年来国家颁布的重要政策文件以及领域专家建议,构建初步医疗物联网水平评价指标体系;②通过两轮德尔菲调查法,邀请领域内15位专家参与评价模型优化工作,并从专家积极系数和专家权威系数两个指标对专家可靠性进行检验,利用协调系数和变异系数检验专家一致性,以确保指标体系的权威性、科学性;③运用层次分析法计算指标体系权重;④采用问卷调查法对湖北省10所医院开展医疗物联网建设情况调查;⑤利用等级评分法、综合评分法完成指标量化,并运用极差值法对所有指标数据进行归一化处理;⑥运用线性加权法计算各调研对象的医疗物联网水平综合得分;⑦通过计算克朗巴赫a系数来检验指标体系的信度,主成分分析法用于检验指标体系的效度。[研究结果]构建一套医疗物联网评价指标体系,包括5个一级指标、20个二级指标和85个三级指标,其中一级指标包括医疗物联网保障体系、医疗物联网基础设施、物联网技术应用、医疗物联网信息安全和医疗物联网标准五个维度,所有指标的专家认可度较高,专家评价通过一致性检验,两轮调查的专家协调系数都在0.5以上,说明评价模型具有一定的权威性和可靠性。通过信度效度检验,显示本指标体系具有较好的一致性信度、内容效度和结构效度,说明本研究的评价模型评价结果是稳定的、可靠的。
[Abstract]:[background] at present, the Internet of things, as a new generation of information technology, is widely used in various fields. In the medical field, the Internet of things technology has the advantages of optimizing the medical service mode and improving the efficiency of hospital operation and management. Although the construction of the medical Internet of things is developing rapidly in China, there is no index system and concrete application for evaluating the level of the medical Internet of things. The construction of medical internet of things is a complex system engineering, the concrete development level and the construction benefit are difficult to grasp subjectively, therefore, it is urgent to have a set of scientific and authoritative evaluation index system. [objective] on the basis of analyzing the present situation and existing problems of medical Internet of things evaluation, a set of scientific authoritative medical Internet of things evaluation system with high reliability and validity was constructed. To promote the construction of the medical Internet of things standardized and scientific. [research methods and contents] 1 through literature analysis, this study looked up the relevant literature on the evaluation of the medical internet of things at home and abroad, and provided the theoretical basis for the evaluation index system. Combined with the important policy documents issued by the state in recent years and the suggestions of experts in the field, the preliminary evaluation index system of the level of the medical Internet of things is constructed. (2) through two rounds of Delphi investigation, 15 experts in the field are invited to participate in the evaluation model optimization, and the expert reliability is tested from two indexes of expert positive coefficient and expert authority coefficient. Using the coordination coefficient and variation coefficient to test the consistency of experts to ensure the authority and scientific nature of the index system; (3) Analytic hierarchy process (AHP) was used to calculate the weight of the index system, and (4) the questionnaire survey was used to investigate the construction of the medical internet of things in 10 hospitals in Hubei Province. (5) the index is quantified by grade scoring method and comprehensive scoring method, and all index data are normalized by extreme difference method, and the comprehensive score of medical internet of things is calculated by linear weighting method. 7 the reliability of the index system was tested by calculating the Cronbach a coefficient, and the validity of the index system was tested by principal component analysis. [results] A set of evaluation index system of the medical Internet of things is constructed, which includes 5 primary indicators, 20 secondary indicators and 85 third-level indicators, among which the first-level indicators include the medical Internet of things guarantee system, the medical Internet of things infrastructure. The application of Internet of things technology, the information security of medical Internet of things and the standard of medical Internet of things are five dimensions, the expert recognition degree of all indexes is high, the expert evaluation passes the consistency test, the expert coordination coefficient of the two rounds of investigation is above 0.5. It shows that the evaluation model has certain authority and reliability. The reliability and validity tests show that this index system has good consistency reliability, content validity and structural validity. The evaluation results of the evaluation model are stable and reliable.
