泰安市儿科护理人员职业认同与离职意愿相关性研究

【摘要】:研究背景随着我国医学模式的转变和医疗市场的激烈竞争,人们对健康的需求和维权的意识不断提高,对护理工作也提出了更高的要求。职业认同是职业人在工作中驰骋的重要心理基础,是发展的内在激励因素。护理人员职业认同感可以影响护士对工作的积极性和满意度,从而影响护理质量。目前,我国护士短缺,较高的护士离职率是引起护士短缺的主要原因之一。离职意愿是离职最有效的预测指标,最终可能导致实际离职的发生。儿科由于其服务对象的特殊性和护理技能的高要求,在工作中要付出更多的精力和时间。儿科护理人员不仅要处理好与患儿及家属的关系,还要处理好与同事间的关系,同时暴力事件还会严重威胁护士的身心健康,导致护士职业认同降低和离职意愿增强。为探索泰安市儿科护理人员职业认同与离职意愿现状及相关因素,对泰安市儿科护理人员进行了现况调查。研究目的本研究旨在通过对泰安市儿科护理人员进行问卷调查,了解儿科护理人员的职业认同现况及离职意愿现况,进一步探索影响儿科护理人员职业认同及离职意愿的相关因素,找出职业认同及离职意愿的相关性,从而提高泰安市儿科护理人员的职业认同,降低其离职意愿,稳定儿科护理队伍。研究对象与方法本研究以泰安市儿科护理人员为研究对象,在取得医院护理部领导的同意和科护士长的支持下,对泰安市3所三级甲等综合医院、2所三级甲等专科医院(中医药系列和儿童系列)及1所二级甲等综合医院共6家医院的儿科护理人员(每家医院儿科护理人员人数在20人以上)发放调查问卷。共发放问卷460份,回收有效问卷442份,有效回收率为96.1%。主要调查指标:儿科护理人员的人口社会学特征、职业认同水平及其离职意愿状况。数据处理采用Excel 2007录入数据,运用SPSS 20.0对资料进行统计学分析。主要结果一、职业认同状况及相关因素研究对象职业认同总分为97.45±16.37,得分率为64.97%;职业认同5个维度得分率由高到低为职业社会支持68.77%,职业挫折应对68.03%,职业自我反思66.80%,职业社交技能62.30%,职业认知评价61.35%。科室、工作年限、聘任方式、医院等级和工作环境满意度5个因素具有统计学意义(P0.05)。二、离职意愿状况及相关因素离职意愿总分为15.66±3.08,其中离职意愿Ⅰ指标值61.28%,离职意愿Ⅱ最低,指标值60.89%,离职意愿Ⅲ最高,指标值73.56%。对儿科护理人员离职意愿总分按照等级划分标准进行划分,离职意愿很低的没有;离职意愿较低的占17.9%;离职意愿较高的占52.3%;离职意愿很高的占29.9%。聘任方式、医院等级、职工自豪感及工作原因4个因素具有统计学意义(P0.05)。三、职业认同与离职意愿的相关性职业认同及各维度与离职意愿、离职意愿Ⅰ、离职意愿Ⅱ均成负相关;职业认知评价与离职意愿Ⅲ成负相关性;职业认同及4个维度与离职意愿Ⅲ无相关性。结论泰安市儿科护理人员职业认同处于中等水平。研究对象对所从事的护理本职工作的概念、意义及价值认知不够深入,对自身的护理人员角色的认知和评价不高。科室、工作年限、聘任方式、医院等级和工作环境满意度是影响儿科护理人员职业认同总体水平的重要因素。泰安市儿科护理人员离职意愿较强,获得外部其他工作可能性较大。聘任方式、医院等级、职工自豪感及工作原因是影响儿科护理人员离职意愿的重要因素。泰安市儿科护理人员职业认同及其5个维度越高,离职意愿越低,辞去当前工作及寻找其他相同或不同性质工作动机也会降低。职业认知评价越高,获得外部工作的可能性越低。
[Abstract]:Background: with the medical mode of our country and the transformation of the medical market competition, awareness of health needs and rights continue to improve, but also put forward higher requirements to the nursing work. Occupation identity is an important psychological basis of human occupation gallop in the work, is the internal incentive factors. The development of nursing staff occupation. The same can affect the enthusiasm and satisfaction of nurses on the work, thus affecting the quality of nursing. At present, China's shortage of nurses, the high turnover rate of nurses is one of the main reasons causing the shortage of nurses. Turnover intention is the most effective predictor of turnover, may eventually lead to the occurrence of actual turnover. Because of the high requirements and special pediatric nursing the skills of service object, in order to spend more time and energy. Nursing staff should not only handle the relationship with the children and their families, but also deal with The relationship between colleagues, and violence but also a serious threat to the physical and mental health of nurses, to reduce the turnover intention of nurses' occupation identity and enhancement. In order to explore the nursing staff in Tai'an city of occupation identity and turnover intention status and related factors of nurses in Tai'an City pediatric for investigation. The purpose of this study is to through questionnaire survey the nursing staff in Tai'an in pediatrics, pediatric nurses to understand the occupation identity and turnover intention status, to further explore the effects of pediatric nurses' occupation identity and related factors of turnover intention, to find out the correlation between occupation identity and turnover intention, so as to improve the occupation identity of nursing staff in Tai'an city children, reduce turnover intention, pediatric nursing team stability the object of study and the method of the study on pediatric nursing personnel in Tai'an city as the research object, made in the hospital nursing department. The consent and the head nurse's support, 3 in Tai'an and three general hospitals, 2 three level of first-class hospital (traditional Chinese medicine series and the series of children) and 1 pediatric nurses in grade two general hospital a total of 6 hospitals (the number of pediatric nurses every hospital in more than 20 people) questionnaire. A total of 460 questionnaires, 442 valid questionnaires, the effective recovery rate as the main investigation index 96.1%.: pediatric nurses' demographic and sociological characteristics, occupation identity level and turnover intention status. Excel 2007 input data used for data processing, statistical analysis of data using SPSS 20. The main results. Occupation identity status and related factors of occupation identity as the research object score 97.45 + 16.37, the score rate was 64.97%; 5 dimensions of occupation identity score rate from high to low as 68.77% occupation of social support, occupation frustration coping 68.03% 66.80%, occupation self reflection, occupation social skills 62.30%, occupation cognition and evaluation 61.35%. departments, work experience, appointment way, with significant levels of satisfaction and working environment of the hospital 5 factors (P0.05). Two, turnover intention status and related factors of turnover intention of the total score was 15.66 + 3.08, the turnover intention of the index value of 61.28%. The turnover intention of the lowest index value of 60.89%, the highest index value of turnover intention, 73.56%. of the pediatric nurses' turnover intention scores are divided according to the grading standards, have very low turnover intention; turnover intention lower accounted for 17.9%; higher turnover intention accounted for 52.3%; turnover intention is very high for 29.9%. appointment, hospital grade, with significant staff pride and work 4 factors (P0.05). Three, occupation identity and turnover intention relationship between occupation identity and dimensions and turnover intention and turnover intention May I, II are negatively related to turnover intention; occupation cognitive evaluation and turnover intention of negative correlation; occupation identity and the 4 dimensions and turnover intention of no correlation. Conclusion the nursing staff of pediatric Tai'an city occupation identity is in the medium level. The research object of concept in nursing work, the significance and value of perception is not enough further, high cognition and evaluation of the nursing role. The Department, working life, employment, the degree of satisfaction and working environment of the hospital is an important factor affecting the overall level of pediatric nurses' occupation identity. Tai'an City pediatric nurses' turnover intention is strong, get outside other work more likely. The appointment way, the rank of the hospital. The pride and work is an important factor to influence turnover intention of nursing staff in pediatrics. Tai'an pediatric nurses' occupation identity and the 5 dimensions of the higher, The lower the turnover intention, the lower the resignation of the current job and the other same or different nature of work motivation. The higher the occupation cognitive evaluation, the lower the possibility of obtaining external work.
