非肿块型乳腺癌超声弹性成像表现与其免疫组化的相关性分析

【摘要】:目的:分析非肿块型乳腺癌的常规超声及其弹性成像特点,结合免疫组化结果,研究其相关性,为非肿块型乳腺癌的早诊断、治疗及其预后提供新的依据。方法:收集30例经过病理证实的乳腺癌患者,超声表现为非肿块型,记录其常规超声表现(位置、大小、边界、血流、阻力指数、回声等);记录其弹性成像应变率比值并进行弹性评分;将病灶分级,结合其弹性成像特点与其免疫组化结果:雌激素受体(estrogen receptor ER)、孕激素受体(progesterone receptor PR)、肿瘤抑制基因(tumor suppressor gene P53)、人类表皮生长因子相关基因2(human epidermal growth factor receptor HER-2),进行相关性分析。结果:弹性评分与ER相关(P0.05);弹性评分与PR相关(P0.05);弹性评分与Her-2无相关性(P0.05);弹性评分与P53相关(P0.05)。弹性评分4-5级组的ER、PR阳性表达率高于、P53阳性表达率低于1-3级组。应变率比值(Strain Ratio SR)与ER相关(P0.05);SR与PR相关(P0.05);SR与Her-2相关(P0.05);SR与P53相关(P0.05)。SR≥3.0组的PR阳性表达率低于、ER、Her-2、P53阳性表达率高于SR3组。结论:1.非肿块型乳腺癌弹性成像特点与其免疫组化因子ER、PR、P53表达具有相关性,为非肿块型乳腺癌治疗及其预后提供帮助。2.弹性评分4-5级组的非肿块型乳腺癌具有较高的ER、PR阳性表达率、较低的P53阳性表达率,预示其更适合内分泌治疗,预后较好。3.SR≥3.0的非肿块型乳腺癌具有较低的PR阳性表达率,较高的ER、Her-2、P53阳性表达率,预示其不适合内分泌治疗,预后不好。
[Abstract]:Objective: to analyze the characteristics of conventional ultrasound and elastic imaging of non-mass breast cancer, and to study its correlation with immunohistochemical results, so as to make an early diagnosis of non-mass breast cancer.
