关于青少年速滑运动员膝关节磁共振定量参数变化的研究

【摘要】:目的:假设速滑运动能引起青少年髌软骨及半月板组织结构的改变,导致MRI定量参数的变化,研究青少年速滑运动员与正常青少年膝关节髌骨软骨与半月板之间T2 mapping、T2 star及Resolve DWI定量参数的差异是否有统计学意义。方法:研究对象:将随机抽取的某中学初二年级某班级的39名学生纳入正常对照组,将随机抽取的某速度滑冰训练基地28名青少年速度滑冰运动员纳入实验组。采用3.0T磁共振(Mgnetom Skyra system,Siemens),膝关节专用线圈,仰卧位,足先进,对研究对象进行扫描。由2位有5年膝关节MRI阅片经验的影像诊断医师采用双盲法进行数据测量,测量所得数据采用SPSS 19.0进行统计学分析。数据进行正态分布检测,若符合正态分布,用平均数±标准差表示,两组间采用独立样本t检验进行统计学分析;若不符合正态分布,用中位数表示,采用非参数检验进行统计学分析。组内两两比较均采用q检验。P㩳0.05认为差异有统计学意义。结果:1.正常组髌软骨T2值(24.02±4.25)、ADC值(1158.49±203.63)低于实验组(29.07±7.17)、(1578.48±294.28),P㩳0.05,差异有统计学意义;正常组髌软骨T2*值(22.71±3.24)低于实验组(23.72±5.95),P0.05,差异无统计学意义。2.正常组内侧半月板前角T2值(18.64±3.88)、ADC值(773.53±272.46)低于实验组(22.92±4.82)、(1178.60±234.80),P㩳0.05,差异有统计学意义;实验组内侧半月板前角T2*值(5.98±2.26)低于正常组(6.83±2.28),P0.05,差异无统计学意义。3.正常组内侧半月板后角T2值(17.37±6.57)、ADC值(741.75±159.19)低于实验组(21.41±3.76)、(1130.44±268.39),P㩳0.05,差异有统计学意义;正常组内侧半月板后角T2*值(7.85±3.30)低于实验组(8.30±2.34),P0.05,差异无统计学意义。4.实验组外侧半月板前角T2值(19.27±4.70)低于正常组(19.55±7.05),正常组外侧半月板前角T2*值(6.39±1.96)、ADC值(734.01±252.72)低于实验组(6.67±1.66)、(857.21±243.89),P0.05,差异无统计学意义。5.正常组外侧半月板后角T2值(15.26±2.47)、T2*值(6.90±1.58)、ADC值(881.00±114.00)低于实验组(17.16±4.24)、(8.54±5.99)、(1026.11±258.75),P0.05,差异无统计学意义。结论:1.青少年速滑运动员与正常青少年的髌软骨T2值和ADC值存在差异。2.青少年速滑运动员与正常青少年的内侧月板前、后角T2值和ADC值存在差异。
[Abstract]:Objective: to assume that speed skating can cause changes in the tissue structure of patellar cartilage and meniscus in adolescents and lead to the change of quantitative parameters of MRI. To study the difference of quantitative parameters of T 2 mapping T 2 star and Resolve DWI between juvenile speed skaters and normal juvenile knee joint patella cartilage and meniscus. Methods: study participants: a middle school junior high school selected randomly. Thirty-nine students in a class in a grade were included in the normal control group. Twenty-eight young speed skaters were randomly selected from a certain speed skating training base to be included in the experimental group with 3.0T magnetic resonance imaging (MRI) of Mgnetom Skyra system Siemens, a special coil for the knee joint, supine position, advanced foot, and so on. Two imaging diagnostics who had 5 years experience in reading MRI film of knee joint were used to measure the data by double blind method. The measured data were analyzed statistically by SPSS 19.0. The data were detected by normal distribution. If the normal distribution is consistent with the mean 卤standard deviation, the independent sample t test is used for statistical analysis between the two groups; if it does not conform to the normal distribution, the median is used. A non-parametric test was used for statistical analysis. Results the T _ 2 value of patellar cartilage in normal group was 24.02 卤4.25 and ADC value was 1158.49 卤203.63), which was lower than that in experimental group (29.07 卤7.17) and 1578.48 卤294.28 (P < 0.05). The T _ 2 * value of patellar cartilage in the normal group was 22.71 卤3.24), which was lower than that in the experimental group (23.72 卤5.95), no significant difference was found. 2. The T _ 2 value of the medial meniscus anterior horn of the normal group was 18.64 卤3.88 卤272.46) and the ADC value of the medial meniscus of the normal group was lower than that of the experimental group (22.92 卤4.82). The T _ 2 * value of the medial meniscus in the experimental group was 5.98 卤2.26), which was lower than that in the normal group (6.83 卤2.28), and there was no significant difference. 3. The T _ 2 value of the medial meniscus in the normal group was 17.37 卤6.57 卤159.19) and the ADC value of the medial meniscus was lower than that of the experimental group (21.41 卤3.760.1144 卤268.39). The difference was statistically significant. The T _ 2 * value of the medial meniscus in the normal group was 7.85 卤3.30), which was lower than that in the experimental group (8.30 卤2.34), the difference was not statistically significant. The T _ 2 value of the lateral meniscus in the experimental group was 19.27 卤4.70), and the T _ 2 * value of the anterior angle of the lateral meniscus in the normal group was 6.39 卤1.96 卤252.72), which was lower than that in the experimental group (6.67 卤1.667.21 卤243.89P0.05), and the ADC value of the lateral meniscus in the normal group was lower than that in the experimental group (6.67 卤1.667.21 卤243.89P0.05). There was no statistical significance. The T 2 value of the lateral meniscus in the normal group was 15.26 卤2.47 T2 * and the ADC value was 881.00 卤114.00, which was lower than that in the experimental group (17.16 卤4.24), which had no statistical significance. Conclusion there is no statistical significance between the T 2 value and ADC value of patellar cartilage between juvenile skaters and normal adolescents. Young speed skaters and normal teenagers in front of the medial meniscus, There was a difference between T _ 2 and ADC in the posterior horn.
