PICC与VPA对于高耐药性肺癌患者化疗中的不良反应影响

【摘要】：目的探讨经外周中心静脉置管(PICC)与植入式中心静脉输液港(VPA)对高耐药性肺癌患者化疗中的不良反应发生情况。方法选择2011年2月至2013年2月哈尔滨医科大学附属第二医院收治诊断为肺癌的患者834例,其中对化疗效果欠佳患者行耐药性分析最终考虑为高耐药性的患者共56例。将56例高耐药肺癌患者分成PICC组和VPA组,每组28例。随访13个月后,比较两组患者的导管平均留置时间、导管相关并发症,非导管相关的临床并发症及生活质量、平均生存时间的差异。结果 VPA组导管平均留置时间及导管相关、非导管相关的并发症发生率均明显低于PICC组(P0.05)。两组患者的生活质量评分比较,差异无统计学意义(P0.05);但VPA组患者生活质量评分A级的比例明显高于PICC组(P0.05)。两组患者的平均生存时间比较,差异无统计学意义(P0.05)。结论 VPA在高耐药性肺癌患者反复多次化疗过程中可作为一种长期留置,此方法方便且安全。
[Abstract]:Objective to investigate the incidence of adverse reactions in chemotherapy of patients with high drug resistance lung cancer treated by PICC and VPA. Methods from February 2011 to February 2013, affiliated to Harbin Medical University, Harbin Medical University was selected. A total of 834 patients with lung cancer were admitted to the second Hospital. 56 patients with high drug resistance were divided into PICC group and VPA group, 28 patients in each group were followed up for 13 months. The differences of average catheter indwelling time, catheter related complications, non-catheter related clinical complications, quality of life and average survival time were compared between the two groups. Results the average catheter retention time and catheter correlation in VPA group were compared. The incidence of non-catheter-related complications was significantly lower than that of PICC group (P 0.05). There was no significant difference (P 0.05), but the proportion of quality of life score A in VPA group was significantly higher than that in PICC group (P 0.05). The mean survival time of the two groups was higher than that of PICC group. Conclusion VPA can be used as a long-term retention during repeated chemotherapy in patients with high resistance lung cancer, and this method is convenient and safe.
