MRI时间-信号强度曲线测量区域选择对乳腺病变的定性诊断价值

【摘要】:目的探讨选择不同区域(即病灶中央区与边缘区)测量MRI时间-信号强度曲线(TIC)对乳腺良恶性病变的鉴别诊断价值。方法搜集经病理证实的乳腺疾病患者322例(良性142例,恶性180例),共358个病灶,术前或活检前行磁共振动态增强扫描(DCE-MRI);测量所有病灶中央区及边缘区TIC类型,统计分析良、恶性病灶中央区与边缘区TIC类型分布有无差异;并采用受试者工作特性曲线(ROC)分析比较病灶中央区及边缘区TIC类型在不同诊断界值下对乳腺良恶性病变的鉴别诊断效能。结果良、恶性病灶中央区与边缘区TIC类型分布差异均存在统计学意义(P0.05)。选择病灶边缘区测量TIC类型且以诊断界值≥3级(即Ⅲ型曲线诊断为恶性)作为恶性病变的诊断标准,其ROC曲线下面积为0.854,且与选取病灶中央区测量TIC类型诊断乳腺恶性病变的曲线下面积有统计学差异(P0.05),诊断敏感度、特异度分别为70.35%、93.08%。结论选择病灶不同区域测量MRI TIC类型对乳腺良恶性病变的诊断效能存在差异,选取病灶边缘区测量TIC类型且以Ⅲ型曲线作为恶性病变的诊断标准,可以提高对乳腺良恶性病变的诊断能力。
[Abstract]:Objective to evaluate the differential diagnosis of benign and malignant breast lesions by measuring MRI time-signal intensity curves in different regions (i.e. central and marginal regions). Methods A total of 322 patients (142 benign and 180 malignant) with breast diseases confirmed by pathology were collected. DCE-MRI was performed on dynamic contrast enhanced magnetic resonance imaging before operation or before biopsy, and TIC types in the central and marginal areas of all lesions were measured, and good results were analyzed statistically. There was no difference in the distribution of TIC types between the central and marginal regions of malignant lesions, and the differential diagnostic efficacy of TIC types in the central and marginal regions of the lesions for benign and malignant breast lesions was analyzed and compared by using the operating characteristic curve (OC). Results there were significant differences in the distribution of TIC between the central and marginal regions of benign and malignant lesions (P 0.05).
