一种B群链球菌临床检测方法的研制

【摘要】:B群链球菌(group B streptococcus,GBS)是一类寄生于人类泌尿生殖道及下消化道的细菌,会导致孕妇流产、胎膜早破等临床症状,此外,也能导致新生儿患败血症、肺炎、脑膜炎等疾病。准确快速地检测和鉴别孕产妇感染B群链球菌的情况具有重要的临床意义。检测B群链球菌所使用的标本为阴道分泌物,分离检测和鉴别用时较长,检测手续繁琐、精度低,目前临床上急需一种准确快速的检测方法。B群链球菌显色培养基在国际已有相关研究,但国内尚未见报道。进口产品价格昂贵、转运周期较长、保存要求高,进而影响其在国内的普遍推广和使用。本论文采用特异性酶底物显色技术,研制一种B群链球菌临床检测方法,以期获得快速准确的检测结果。研制方法和步骤主要为:以营养琼脂培养基为基础,后通过补加营养物质和生长因子,以灵敏度和特异性标准菌株的生长情况为指标,筛选出培养基的基础配方,再结合特异性酶显色底物等,通过一系列优化试验和正交试验,筛选出B群链球菌显色培养基的最佳配方;最后分别用20株临床分离的B群链球菌和200份临床疑似标本,以科马嘉B群链球菌显色培养基为对照,评价该配方培养基的性能。结果表明:1)通过对基础培养基和显色培养基的研制和优化,确定B群链球菌显色培养基的主要成分,即琼脂粉、蛋白胨、酵母浸粉、牛心浸粉、海藻糖、特异性酶显色底物、多粘菌素B、两性霉素B、庆大酶素、吐温80、氯化高铁血红素等,p H值为7.5;2)采用临床分离的B群链球菌菌株,与科马嘉显色培养基对比,二者检出率均为100%;3)在对临床疑似标本检测中,本品性能优于科马嘉显色培养基检测方法,科马嘉的检测灵敏度为85.8%,特异性为92.3%,而本品的两项指标均为100%。Vitek2 System鉴定系统检测结果支持本品的结论。本论文采用特异性酶显色底物技术,研制的B群链球菌临床检测方法可获得快速准确的检测结果,且性能优于法国科马嘉的同类产品,可用于临床检测。
[Abstract]:Group B Streptococcus (group B Streptococcus (GBS)) is a class of bacteria parasitic on the human urogenital tract and the lower digestive tract. It can cause pregnant women to abort, premature rupture of membranes and other clinical symptoms, in addition, it can also cause neonatal sepsis, pneumonia, meningitis and other diseases. Accurate and rapid detection and identification of pregnant and lying in and lying in women with group B Streptococcus are heavy. Clinical significance. Detection of group B Streptococcus specimens for vaginal secretions, separation detection and identification used longer, tedious procedures, low accuracy, the clinical need for an accurate and rapid detection method.B group streptococcus color culture medium has been related research in the world, but there is no report at home. The price of imported products is expensive, In this paper, a specific enzyme substrate chromogenic technique is used to develop a clinical detection method of group B Streptococcus, in order to obtain rapid and accurate detection results. The methods and steps are mainly based on the nutrient agar medium and then by supplementing camp. Material and growth factors were used to screen out the basic formula of the medium and the specific enzyme chromogenic substrate. The best formula of group B streptococcal color culture medium was screened out by a series of optimization tests and orthogonal experiments. Finally, 20 B group streptococci were used to separate the clinical isolates of Streptococcus. The bacteria and 200 clinically suspected specimens were compared with the chromogenic medium of group B Streptococcus. The results showed that: 1) the main components of the color culture medium of group B Streptococcus, namely, agar powder, peptone, yeast extract, cow heart dipping powder, trehalose, were determined by the development and optimization of the basic culture medium and color culture medium. The specific enzyme chromogenic substrate, polymyxin B, amphotericin B, Qing Da enzyme, Twain 80, high hemin chlorination, P H value 7.5; 2) the clinical isolation of group B Streptococcus, compared with the Colma coloured culture medium of the Colma color culture, the two detection rates were 100%; 3) in the detection of the suspected specimen of the bed, the quality of this product was superior to the Colma coloration culture basis. The sensitivity of the test was 85.8% and the specificity was 92.3%. The two indexes of this product were all the results of the 100%.Vitek2 System identification system. This paper uses the specific enzyme chromogenic substrate technology to develop the clinical detection method of group B Streptococcus, and the performance is better than the method. Congener's similar products can be used for clinical testing.
