基层医疗机构护士从事养老医疗护理意愿及其影响因素与促进对策

【摘要】:研究目的:伴随着国务院《关于加快发展养老服务业的若干意见》的颁布以及日益快速发展的人口老龄化形势,养老医疗护理服务已成为最为重要的养老服务内容之一。我国基层医疗机构护士均接受过专业的医疗护理教育与培训,其医疗护理知识、技能与临床经验均完全能够胜任养老医疗护理工作。因此,掌握目前基层医疗机构护士从事养老医疗护理服务的意愿现状,深入挖掘其影响因素,并据此提出相应的对策建议,从而在一定程度上有助于解决养老医疗护理人才不足的问题,促进我国养老医疗服务质量的提升,以更好地满足老龄化社会需求。研究内容:本文首先通过前人研究和回顾政府有关促进养老医疗护理事业发展规划等方面的政策文件,界定了养老医疗护理的概念,了解养老医疗护理人员工作性质与要求,并且从国内外养老医疗护理人才从业意愿及其影响因素研究现状,发现其不足之处,以寻找本研究的突破口。其次,基于计划行为理论解释与实地访谈的基础上,提出本研究假设并构建模型,对基层医疗机构护士从事养老医疗护理意愿的影响因素进行了实证分析和假设验证。研究基层医疗机构护士不同人口学变量在其意愿水平上的差异,讨论从事养老医疗护理意愿及其影响因素的相关性,为提高基层医疗机构护士从事养老医疗护理意愿水平提供相关对策。研究方法:现场调研选取地点为经济发达且老龄化现象严重,迫切需要养老医疗护理服务的杭州和南京地区。采用随机分层抽样方法,抽取30家基层医疗卫生机构,对300名基层医疗卫生机构护士进行问卷调查,对获取的数据进行分析和假设检验。研究结论:不同年龄、工作年限、用工形式、职称的基层医疗机构护士意愿水平存在显著差异(P0.05,P0.01)。基层医疗机构护士从事养老医疗护理意愿水平受发展机会、感知工作态度、工作内容评价、管理制度评价、他人或社会环境影响等多方面的影响。因此,应增强基层医疗机构护士从事养老医疗护理的积极意识;政府积极引导,加大政策优惠力度;制定出规范的养老医疗护理管理制度,建立责权分明的协调体系;通过政府、基层医疗机构以及护士自身职业理念这三方面共同发挥专业技能优势,提供新的发展平台,推动更多基层医疗机构护士参与养老医疗护理服务。
[Abstract]:Objective: with the promulgation of the State Council's opinions on accelerating the development of the old-age service industry and the aging of the population, the medical and nursing services for the aged have become one of the most important services for the aged. Nurses in primary medical institutions in China have received professional education and training in medical care, and their medical and nursing knowledge, skills and clinical experience are all fully qualified for the work of old-age medical care. Therefore, we should grasp the present situation of nurses' willingness to engage in old-age medical care service in primary medical institutions, dig into its influencing factors, and put forward corresponding countermeasures and suggestions accordingly. To a certain extent, it will help to solve the problem of shortage of medical and nursing talents for the aged, promote the improvement of the quality of medical service for the aged in our country, and better meet the needs of the aging society. Research content: first of all, through the previous research and review of the government policy documents on promoting the development of medical care for the aged, this paper defines the concept of medical care for the aged, and understands the nature and requirements of the work of the medical and nursing personnel for the aged. And from the domestic and foreign endowment medical and nursing talent employment intention and its influence factor research present situation, discovers its insufficiency, in order to seek the breakthrough point of this research. Secondly, based on the explanation of planning behavior theory and field interviews, this paper puts forward the hypothesis of this study and constructs a model, which makes empirical analysis and hypothesis verification on the influencing factors of nurses' willingness to take part in old-age medical care in primary medical institutions. To study the differences of demographic variables of nurses in primary medical institutions on their willingness level, and discuss the correlation between the willingness of nursing care for the aged and its influencing factors. In order to improve the level of nurses' willingness to take part in old-age medical care in primary medical institutions, the relevant countermeasures were provided. Methods: field investigation was conducted in Hangzhou and Nanjing, where the economy was developed and the aging phenomenon was serious, and there was an urgent need for medical and nursing services for the aged. The random stratified sampling method was used to select 30 primary medical and health institutions. 300 nurses in primary health care institutions were investigated by questionnaire, and the data obtained were analyzed and hypothetically tested. Conclusion: there are significant differences in the willingness of nurses in primary medical institutions with different ages, working years, employment forms and professional titles. The level of nurses' willingness to take part in old-age medical care is influenced by development opportunity, perceived work attitude, evaluation of work content, evaluation of management system, and influence of others or social environment. Therefore, we should strengthen the positive consciousness of nurses in primary medical institutions engaged in old-age medical care, actively guide the government, increase the intensity of preferential policies, formulate a standardized management system of medical care for the aged, and establish a coordinated system with clear responsibilities and powers. Through the three aspects of government, primary medical institutions and nurses' own professional concept, we can give full play to the advantages of professional skills, provide a new development platform, and promote more nurses in primary medical institutions to participate in old-age medical care services.
