以梯度平板法改进药敏试验方法及其药物浓度质量控制

【摘要】:药敏试验是临床选择敏感抗生素的精确检测工作,其成败在于质量控制的可靠性和最低抑菌浓度(MIC)的精确性、前瞻性.本文结合质控试验和药敏试验对梯度平板法进行验证,梯度平板法所测得的MIC与微量肉汤稀释法和琼脂稀释法测得的MIC结果一致,且因为线性设置的药物浓度,MIC更加精确,能推算抗菌药物的原始浓度,更好地保证质控试验的可靠性.同时,按照美国临床实验室标准委员会或欧盟药敏试验委员会的敏感(S),中介(I)以及抗药(R)范围标准设置0-S,S-R,R-8R这样3个线性梯度,首先判断菌株敏感性范围,然后精确测定细菌群落的MIC值,并能有效分离出抗性菌株,预警该抗生素使用后可能出现的抗药性,进而指导临床抗菌药物的选择和应用.梯度平板的主要困难在于手工制备的繁琐和不稳定性,如果自动化生产即用型梯度平板,将便利临床药敏试验,改进抗药性筛选技术.
[Abstract]:Drug sensitivity test is an accurate test for clinical selection of sensitive antibiotics. Its success or failure lies in the reliability of quality control and the accuracy and prospective of minimum inhibitory concentration (MIC). In this paper, the quality control test and drug sensitivity test were combined to verify the gradient plate method. The MIC determined by gradient plate method was consistent with that of the micro broth dilution method and the Agar dilution method, and because the linear setting of drug concentration and MIC was more accurate, the results obtained by the gradient plate method were in good agreement with those obtained by the micro broth dilution method and the Agar dilution method. The original concentration of antimicrobial agents can be calculated and the reliability of quality control test can be better guaranteed. At the same time, according to the sensitivity (S), intermediary (I) and drug resistance (R) range standards of the American Committee for Clinical Laboratory Standards or the European Commission for Drug sensitivity testing, three linear gradients such as 0-S S-RN R-8R were set up to determine the sensitivity range of the strain. Then the MIC value of bacterial community was determined accurately, and the resistant strain could be effectively isolated, and the resistance of the antibiotic could be warned, and the selection and application of clinical antimicrobial agents could be guided. The main difficulty of gradient plate is the complexity and instability of manual preparation. If the gradient plate is produced automatically, it will facilitate the clinical drug sensitivity test and improve the screening technique of drug resistance.
