ICU护士预防呼吸机相关性肺炎护理的知信行水平及影响因素研究

【摘要】：[目的]调查了解重症监护病房(Intensive Care Unit,ICU)护士对预防呼吸机相关性肺炎(Ventilator-AssociatedPneumonia,VAP)护理的知识、态度/信念、行为现状,探索ICU护士实施预防VAP护理的影响因素,以提高ICU护士 VAP预防护理的依从性,为增强ICU护士在预防VAP方面的继续教育及培训提供参考性建议。[方法]采取方便抽样的方法,于2016年9月至10月对江苏省南京市三级甲等医院的ICU护士进行问卷调查。问卷由ICU护士的一般人口学资料调查表、ICU护士预防VAP护理的知信行水平调查表两部分组成。分别使用Epi Data3.1和SPSS22.0统计软件对数据进行录入与分析。对一般资料采用中位数、平均数、标准差、构成比等进行统计描述;对ICU护士预防VAP护理知识、态度/信念和行为的影响因素使用单因素分析、t检验、多元逐步回归分析、χ2检验、Pearson相关分析等统计学方法进行分析。[结果]1.自制的《ICU护士预防呼吸机相关性肺炎护理的知信行水平调查表》具有较好的信度和效度。2.ICU护士 VAP预防护理知识满分为18分,总均分为10.36±3.20分;态度/信念得分满分为35分,总均分为23.76±6.22分;行为得分满分为55分,总均分为36.58±8.90分。3.ICU护士 VAP预防护理的知识和态度/信念之间、态度/信念和行为之间、知识和行为之间均具有显著相关性(r=0.472,P0.001;r=0.763,P0.001;r=0.503,P0.001),知识、态度/信念、行为三者之间互成正相关。4.单因素分析结果表明:一般情况中的年龄、有无专科资质、ICU工作年限、职称、婚姻状况等因素对ICU护士关于VAP预防护理的知识掌握情况影响具有统计学意义(P0.05);一般情况中的年龄、有无专科资质、能级、ICU工作年限、工作年限、职称、第一学历、婚姻状况等因素对ICU护士关于VAP预防护理的态度/信念影响具有统计学意义(P0.05);一般情况中的年龄、有无专科资质、能级、ICU工作年限、工作年限、职称、婚姻状况等因素对ICU护士关于VAP预防护理的行为依从性影响具有统计学意义(P0.05)。5.多元线性回归分析结果表明:ICU工作年限进入知识回归方程,ICU护士在ICU的工作年限对知识掌握情况有影响;婚姻状况和ICU工作年限进入态度/信念回归方程,ICU护士的婚姻状况和ICU工作年限对态度/信念有影响;ICU工作年限进入行为回归方程,ICU护士在ICU的工作年限对行为依从性有影响。[结论]1.通过本次调查发现,ICU护士对预防VAP护理知识欠缺,态度/信念和行为仍需进一步巩固和提高。2.影响ICU护士预防VAP护理知识的主要因素是ICU工作年限;态度/信念方面的主要影响因素是婚姻状况和ICU工作年限;行为方面的主要影响因素是ICU工作年限。3.应加强ICU护士对预防VAP护理知识的培训和学习,巩固其科学规范执行指南的态度,强化实施VAP预防护理的行为。4.ICU护士预防VAP护理知识与态度/信念之间存在显著相关性、态度/信念与行为之间存在显著相关性、知识与行为之间存在显著相关性。
[Abstract]:[objective] to investigate the knowledge, attitude / belief and behavior of nurses in intensive care unit (Intensive Care Unit,ICU) in preventing ventilator-associated pneumonia (Ventilator-AssociatedPneumonia,VAP), and to explore the influencing factors of ICU nurses in implementing preventive VAP nursing care. In order to improve the compliance of VAP preventive nursing of ICU nurses and provide reference suggestions for further education and training of ICU nurses in the prevention of VAP. [methods] the ICU nurses of Grade 3A Hospital in Nanjing, Jiangsu Province were investigated by the convenient sampling method from September to October 2016. The questionnaire consists of two parts: the general demographic information questionnaire of ICU nurses and the level of knowledge, belief and behavior of nurses in VAP prevention. Epi Data3.1 and SPSS22.0 statistical software are used to input and analyze the data. The median, average, standard deviation and composition ratio were used to describe the general data, and univariate analysis was used to test the influencing factors of ICU nurses' preventive VAP nursing knowledge, attitude / belief and behavior. Multiple stepwise regression analysis, 蠂 2 test and Pearson correlation analysis were used. [result] 1. Self-made questionnaire on the level of knowledge, belief and behavior of ICU Nurses in Prevention of Ventilator-Related pneumonia. 2. The full score of VAP preventive nursing knowledge of nurses in ICU was 18, the total score was 10.36 卤3.20, the full score of attitude / belief was 35, the total score was 10.36 卤3.20, and the full score of attitude / belief was 35. The total score was 23.76 卤6.22, the full score of behavior was 55, the total score was 36.58 卤8.90. 3.The knowledge and attitude / belief, knowledge and behavior of nurses in ICU were significantly correlated with each other (rr 0.472n P0.001P0.001P0.001r0.503P0.001). Attitude / belief and behavior are positively related to each other. The results of univariate analysis showed that: in general, the age, the number of years of working in ICU, the professional title, The influence of marital status on the knowledge of VAP prevention and nursing of ICU nurses was statistically significant (P0.05). The influence of marital status on the attitude / belief of ICU nurses on VAP preventive nursing was statistically significant (P0.05). Marital status and other factors on ICU nurses about VAP preventive nursing behavior compliance has statistical significance (P0.05). 5. The results of multiple linear regression analysis showed that the number of years worked in ICU had an effect on the knowledge acquisition. Relationship between marital status and ICU working years entering attitude / belief regression equation. The marital status of nurses and the working years of ICU have influence on attitude / belief. The regression equation of ICU working years into behavior regression equation suggests that the working years of ICU nurses in ICU have an effect on behavior compliance. [conclusion] 1. Through this investigation, we found that nurses in ICU lack knowledge of VAP, and attitude / belief and behavior still need to be further strengthened and improved. 2. The main influencing factors of ICU nurses' knowledge of preventive VAP were the working years of ICU, the main factors of attitude / belief were marital status and ICU's working years, and the main influencing factors of behavior were ICU's working years. ICU nurses should strengthen their training and study on preventive VAP nursing knowledge, consolidate their attitude towards implementing scientific and normative guidelines, and strengthen the behavior of implementing VAP preventive nursing. 4. There is a significant correlation between VAP nurses' knowledge of preventive VAP nursing and their attitudes / beliefs. There was significant correlation between attitude / belief and behavior, and between knowledge and behavior.
