妊娠期糖尿病孕妇尿路感染病原菌分布与危险因素分析

【摘要】:目的观察妊娠期糖尿病(GDM)孕妇尿路感染的病原菌分布情况及相关危险因素,为降低感染及临床治疗提供参考。方法选取医院2012年5月-2016年6月收治的1 020例GDM孕妇,对其中发生尿路感染的187例患者进行回顾分析,分析病原菌分布及危险因素。结果 1 020例妊娠期糖尿病孕妇共发生尿路感染187例,感染率18.33%;共检出237株病原菌,以革兰阴性菌为主,共173株占73.00%;经logistic回归分析显示,年龄≥30岁、空腹血糖≥8mmol/L、糖化血红蛋白≥7%、24h尿蛋白≥80mg、血尿素氮≥6mmol/L、合并≥2种其他慢性疾病和未使用抗菌药物是妊娠期糖尿病孕妇尿路感染的独立危险因素(P0.05)。结论 GDM孕妇尿路感染的主要病原菌为革兰阴性菌,且该类患者尿路感染的危险因素较多,应根据患者病情合理选择抗菌药物,同时进行及时地治疗。
