初次膝关节表面置换术后切口皮温测定和冰敷的临床效果

【摘要】:目的探讨膝关节置换(TKA)术后正常情况下切口局部早期的皮温变化规律及冰敷治疗的初步临床疗效。方法选择接受单侧初次膝关节表面置换术的患者共281例。随机分为对照组140例,观察组141例,对照组予以常规护理和皮温测定,观察组在此基础上术后予以持续冰敷治疗12周。对所有患者进行术后前5 d,第14天,第6周和第12周双膝关节皮温的监测,观察两组患者皮温、疼痛评分及膝关节功能恢复情况。结果获得随访281例,其中女性172例,男性109例,平均年龄(74.2±6.39)岁,随访时间第1天~第12周。对照组平均皮温差:术后第3天为(3.2±0.4)℃,第5天为(3.0±0.3)℃,第14天为(2.7±0.5)℃,第6周为(2.6±0.2)℃,第12周为(2.5±0.2)℃。观察组平均皮温差:术后第3天为(1.2±0.4)℃,第5天为(1.1±0.3)℃,第14天为(1.2±0.5)℃,第6周为(1.0±0.2)℃,第12周为(0.9±0.2)℃。观察组术后各个监测时间点皮温显著低于对照组(P0.05)。两组患者术后第6周、第12周的VAS评分均较术前明显降低(P0.05),且观察组患者同期VAS评分低于对照组,差异有统计学意义(P0.05)。结论 TKA术后切口周围皮温是临床评估膝关节恢复情况的参考指标。膝关节置换术患者予以早期冰敷治疗,可有效减轻疼痛,减低切开局部皮温,促进膝关节功能尽快恢复。

Methods 281 patients undergoing unilateral primary knee arthroplasty were selected. 140 cases in the control group and 141 cases in the observation group were randomly divided into control group (n = 141) and control group (n = 141). On this basis, the observation group was treated with continuous ice compress for 12 weeks. The skin temperature of double knee joint was monitored 5 days before operation, 14 days, 6 weeks and 12 weeks after operation. The skin temperature, pain score and knee joint function recovery were observed in the two groups.
