单源双能CT平扫直方图分析评估肾透明细胞癌(ccRCC)分级的作用

研究背景和目的:肾癌是一种具有侵袭性和化疗耐药性的恶性肿瘤,约占成人所有肿瘤的3%。肾透明细胞癌是肾细胞癌最常见的类型。约有25%的肾细胞癌患者在确诊时为肿瘤进展期,并且伴有转移的肾细胞癌患者的预后尤其差,生存中值仅为13个月。肾透明细胞癌的临床表现变化很大,从缓慢生长的局限性肿块发展到侵袭性转移性肿瘤。肾切除术合并术后放化疗是对肾透明细胞癌局限性肿块唯一合适的治疗方法。随着我们对肾透明细胞癌遗传因素认识的增加,对肾透明细胞癌的治疗已经转变为肿瘤的靶向识别和治疗的改进。对VEGF和m TOR的理解是肾透明细胞癌生物学的核心,实现了多种抗血管生成药物(舒尼替尼,索拉非尼,帕唑帕尼,依维莫司和贝伐单抗加干扰素-α)在肾透明细胞癌治疗方面的使用。此外,在预后方面,I期肾透明细胞癌患者的预后最好,5年生存率约为80-95%[35]。随着级别的增高,生存率逐渐减低,II期患者的生存率低于80%,III期患者的生存率低于60%。即使进行针对性治疗来抑制肿瘤的进展,IV期患者的生存率也仅有两年多[36]。大多数肾肿瘤是因其他原因进行医学影像检查而偶然发现的[37]。评估肾脏肿块的最常用影像技术是超声...
