头颅CT片个人识别研究

【摘要】: 【研究背景及目的】根据国际刑警组织《遇难者识别指南》,对遇难者的能够用于比较的一切数据都应该得到重视。对于那些能收集到生前CT片的遇难者,结合死后CT片的比对来进行同一认定,是进行个人识别的一种重要方法。以往有很多对头颅X片测量进行个人识别的研究,但由于X线影像有一定程度的放大,其结果准确性差,而CT片却可以准确的测量,可以获得准确的结果,所以通过比较CT片进行个人识别是一个比较好的方法.本研究是通过对头颅CT的数据测量(包括额窦、筛窦、蝶窦等部位),探寻头颅CT片多元识别指标与方法,以运用于法医学实践。 【方法】按照纳入标准收集头颅CT片,测量本次研究设计头颅CT各项指标。用二分类logistic回归分析指标进行性别判定情况。计算相同检查者不同次头颅CT片的各项测量指标差值,再计算不同检查者相同测量指标间的差值,运用二分类logistic回归分析各项指标差值进行头颅CT片同一认定。 【结果】(1)运用颧弓间距指标进行性别识别正确率较高,达87%。而其余各指标进行性别识别准确率不高,综合利用颧弓间距、蝶窦长径、前床突间距、前床突颅内宽径和筛板长度进行性别识别,准确率可以达到90%以上。(2)建立头颅性别判定方程2个。(3)运用额窦宽径指标进行同一认定正确率最高,达93.5%,效果最好;其余各项指标:颧弓间距、乙状窦间距、蝶窦宽径、眼框间距和前床突颅内宽径应用在头颅CT同一认定准确率也在80%以上,有实践价值。(4)建立头颅CT同一认定方程46个。 【结论】(1)运用头颅CT片可以进行有效的性别判定,,并建立头颅性别识别方程2个。(2)运用头颅CT片可以进行有效的个体同一认定,并建立头颅CT同一认定方程共46个。

It is an important method of personal identification for those who can collect pre-mortem CT films and compare postmortem CT films for the same identification. In the past, there were many studies on personal recognition of skull X-ray measurement, but because of the magnification of X-ray images to a certain extent, the accuracy of the results is poor, but the CT film can accurately measure and obtain the accurate results. Therefore, it is a better method to identify individuals by comparing CT films. This study is to explore the multiple identification indexes and methods of skull CT films by measuring the CT data (including frontal sinus, ethmoid sinus, sphenoid sinus, etc.). To apply to forensic practice. [methods] CT films were collected according to inclusion criteria, and the indexes of CT were measured. The gender was determined by binary logistic regression analysis. The difference of each measurement index of different head CT films of the same examiners was calculated, and the difference of the same measurement indexes of different examination subjects was calculated. The difference of each index was analyzed by logistic regression analysis of two categories to confirm the same index of the skull CT film. [results] (1) the correct rate of sex identification was 87% by using zygomatic arch spacing index. However, the sex identification accuracy of other indexes was not high. Sex identification was carried out by using zygomatic arch spacing, sphenoid sinus length diameter, anterior clinoid process spacing, cranial wide diameter and ethmoidal plate length of anterior bed process. The accuracy rate could be more than 90%. (2) two head sex determination equations were established. (3) the highest accuracy rate of 93.5% was achieved by using frontal sinus width and diameter index, and the best effect was achieved. Other indexes: zygomatic arch spacing, sigmoid sinus spacing, sphenoid sinus wide diameter, eye frame space and anterior bed process skull width diameter were also more than 80% accurate in the same identification of head CT. It has practical value. (4) 46 identified equations of skull CT were established. [conclusion] (1) effective sex determination can be carried out by using skull CT film, and two brain sex identification equations can be established. (2) effective individual identity can be determined by using skull CT film. A total of 46 identified equations of skull CT were established.
