SCD心脏ANP与BNP的免疫组织化学研究

【摘要】:背景及目的:SCD 检验鉴定是法医病理学中的常见问题之一。心肌缺血被认为是 SCD 的重要原因,但 SCD 肉眼检查及常规组织病理学检查心肌梗死灶检出率很低,寻找可在心肌组织上灵敏表达的早期心缺血指标,成为 SCD 研究课题的重点及难点。但对可导致 SCD 发生的非缺血因素以及 SCD 发生机制却研究很少。ANP、BNP 属心脏的神经内分泌激素,对循环血量、电解质、血管等具有调节作用,其血浆水平能反映心脏功能状况,在可导致 SCD 的多因素(缺血性和非缺血性)中,利钠肽均具有良好相关性。本实验利用尸检心肌标本,应用免疫组织化学方法检测 ANP、BNP 在 SCD 组心肌、非 SCD 心肌、非 SD 正常心肌中的表达情况,初步探讨 NP 在法医学诊断 SCD 的价值。 方法:1.选材: 选取法医教研室 2000 年至 2004 年尸检心脏标本36 例,其中男性 21 人,女性 15 人,平均年龄 44.9 岁,(具体年龄构成见表 1)按以下标准分为 3 组:(1).SCD 组 12 例(A 组),其中冠状动脉硬化性心脏病(冠状动脉Ⅲ°以上狭窄)未伴明显梗死灶 4 例,心肌病 4 例,心肌炎 4 例,以上均经肉眼和显微镜同时证实,符合诊断标准,其它系统没有检见严重器质性疾病,法医病理学诊断为 SCD;(2)非 SCD组 12 例(B 组),其中脑出血 4 例, 肺栓塞 4 例,急性腹膜炎 4 例,其中在肉眼和显微镜中心脏均未检见较重器质性疾病.在(1)(2)两组,均为平素身体貌似健康,突然发病所致,从发病到死亡在 24 小时以内.(3)非 SD正常心肌组 12 例(C 组),即因心脏外疾病死亡,从发病到死亡在 48 小
