【摘要】:背景忠诚一词可以被定义为表征人们在感觉、情感或习惯上始终如一的附属物。2004年法国引入"全科医生宣言(首选医生宣言)",因为其需要通过国家医疗保险实现医疗服务的最优覆盖,所以法国采用了一种正式激励机制来鼓励患者忠实于他们自己的医生。关于评估该措施的影响并确定医患间忠诚度的组成部分的研究还是一个空白。目的探讨如何建立并维持患者对其全科医生的忠诚度。设计与场所基于对临近法国巴黎地区(伊夫林省的行政部门)的半结构化访谈,进行定性研究。方法在5个具有不同人口统计学特征的个体经营的全科医生的诊疗室中,对28例患者进行访谈。转录访谈内容,并对数据进行主题分析及分类。采用现象学分析法分析转录本。结果患者忠诚度主要基于信任。信任可以通过某些令人欣慰的因素得以加强,如倾听的能力、细心的意识和护理的质量。忠诚既是一种动态构建,又是一种关系的转换,受多种因素影响。患者发现保持忠诚的优势。"家庭医生"的模式一直是一个家族几代人对于患者忠实于自己的医生的原型。全科医生不具备满足患者所有要求的能力不一定是打破患者忠诚的决定因素。结论患者对于医生的忠诚度比通常假设的要复杂的多,其涉及信任、倾听、护理质量,有效性和熟悉程度等多个维度。从本研究中得出的观察结论值得进行更大规模的调查。
[Abstract]:The word background loyalty can be defined as a representation of people feeling, Affective or habitually consistent appendages. In 2004 France introduced the General Practitioners' Declaration (the Declaration of doctors of first Choice) because it requires optimal coverage of health services through national health insurance. Therefore, France has adopted a formal incentive mechanism to encourage patients to be faithful to their own doctors. There is still no research on assessing the impact of the measure and determining the components of patient-doctor loyalty. How to establish and maintain patients' loyalty to their general practitioners. Design and location are based on semi-structured interviews with the administrative department in the Evelyn region, near Paris, France. Methods 28 patients were interviewed in 5 self-employed general practitioners with different demographic characteristics. Subject analysis and classification of the data. Analysis of transcripts by phenomenological analysis. Results patients' loyalty is mainly based on trust. Trust can be enhanced by some comforting factors, such as listening ability. Careful consciousness and the quality of care. Loyalty is both a dynamic construction and a transformation of relationships, The pattern of "family doctor" has always been the prototype of a family of generations loyal to their own doctor. General practitioners do not have to meet all the needs of patients. Ability is not necessarily a determinant of patient loyalty. Conclusion patients' loyalty to their doctors is much more complex than is usually assumed. It involves multiple dimensions of trust, listening, quality of care, effectiveness and familiarity. The findings from this study merit a larger survey.
