【摘要】:研究目的:建立糖尿病大鼠胫骨骨缺损模型,通过比较使用神经生长因子(nerve growth factor,NGF)、羟基磷灰石(hyzdroxya patite,HA)以及两者联合运用后对骨缺损愈合过程中对大体形态学、影像学、碱性磷酸酶(alkaline osphatase,ALP)、骨痂中Ca含量、组织学等指标的影响,探讨联合运用神经生长因子及羟基磷灰石对糖尿病大鼠骨缺损的疗效与机理。 材料和方法: 75只健康雄性8周龄SD大鼠,随机分为链脲佐菌素(streptozotoein,STZ)注射组60只,正常对照组15只,STZ组一次性腹腔注射链脲佐菌素(streptozotoein,STZ)50mg/kg,正常组作为一次性腹腔注射等容量生理盐水,STZ组注射STZ后3天后随机选出血糖浓度高于16.8mmol/L的纳入糖尿病组,血糖未达标者再补充。将糖尿病组随机分为B、C、D、E四组,每组15只。正常组随机选15只血糖正常者为A组正常对照组。5组大鼠均建立右胫骨上段4mm骨缺损模型,按A、B、C、D、E字母顺序分别于骨缺损局部使用,生理盐水、生理盐水、NGF、HA、NGF\HA。术后第4周、8周、12周各组分别随机抽取5只大鼠静脉采血.进行大体标本、骨缺损部位X线、血清ALP检测、骨痂Ca含量及组织学观察。 结果:术后同一时间,大体观察:糖尿病组B、C、D、E组局部骨缺损处新生骨组织体积、硬度逐渐增加,空白对照A组愈合优于糖尿病对照B组,差于D、E组;X线片示:观察B、C、D、E组骨缺损骨痂横向与纵向模糊程度逐渐增加,空白对照A组愈合优于糖尿病B、C组,差于D、E组。组织学观察:B、C、D、E组骨小梁成熟度和密度逐渐增高,空白对照A组愈合优于糖尿病B、C组,低于其余组。B、C、D、E组碱性ALP含量依次递增。A、C组于第4周P0.05,无统计学意义。各组的血清ALP含量于第8周增加达到最高峰,其中E组最高,与其它四组比较血清中ALP含量差异具有统计学意义(P0.05),,ALP于12W回落。各组大鼠新生骨痂Ca含量随时间增加而增加,横向对比各时期由小到大B、C、A、D、E。 结论: NGF与HA联合运用具有良好的诱导成骨作用,使骨细胞增生活跃,加快糖尿病鼠骨缺损愈合。
[Abstract]:Objective: to establish a model of tibial bone defect in diabetic rats and to compare the use of nerve growth factor (NGF) with nerve growth factor (NGF). Hydroxyapatite hyzdroxya patitehae) and the combined use of both in the healing process of bone defects in the gross morphology, imaging. The effect of alkaline phosphatase (ALP) on the contents of Ca and histology in callus. To explore the therapeutic effect and mechanism of nerve growth factor and hydroxyapatite in treating bone defect in diabetic rats. Materials and methods: Seventy-five healthy male 8-week-old SD rats were randomly divided into streptozotoein STZ (STZ) injection group (n = 60) and normal control group (n = 15). Streptozotoein group (STZ group) was injected intraperitoneally with 50 mg / kg streptozotoein (STZ) and the normal group was treated as a single intraperitoneal injection of normal saline of equal volume. Three days after the injection of STZ, the STZ group was randomly selected as the diabetic group with blood glucose concentration higher than 16.8 mmol / L, and those who did not meet the standard of blood sugar were supplemented. The diabetic group was randomly divided into four groups. 15 rats in each group. 15 normal subjects were randomly selected as group A normal control group (group A). 4 mm bone defect model of right upper tibia was established in group A. The alphabetical sequence of E was used locally in bone defect, saline, normal saline, NGFHANGF\ HA. the 4th week after operation was 8 weeks. At 12 weeks, 5 rats were randomly selected for venous blood collection. The gross specimens, bone defect X-ray, serum ALP, callus Ca content and histology were observed. Results: at the same time after operation, gross observation: the volume and hardness of the new bone tissue in the local bone defect of the diabetic group were gradually increased, and the healing of the blank control group A was better than that of the diabetic control group B. The difference was worse than that in group E. X-ray examination showed that the degree of transverse and longitudinal ambiguity of bone defect callus was gradually increased in group A, and the healing of group A was better than that of group B C of diabetes mellitus, which was worse than that of group DX E. the histological observation showed that the bone defect in group A was better than that in group B (n = 6). The bone trabecular maturation and density increased gradually in group D, and the content of alkaline ALP in group A was higher than that in group A, which was higher than that in group B and C of diabetes mellitus, and the content of alkaline ALP in group D was higher than that in group B (P < 0.05). Group C at the 4th week P0.05, no statistical significance. The serum ALP content in each group reached the highest peak at the 8th week, among which group E was the highest. Compared with the other four groups, the serum ALP content was significantly lower than that of the other four groups (P 0.05). The Ca content of the callus increased with the increase of time. The horizontal comparison is made from small to large, from small to large, from small to large. Conclusion: the combination of NGF and HA can induce osteogenesis, activate the proliferation of bone cells and accelerate the healing of bone defect in diabetic rats.
