犬类房颤模型中的心脏三维ECG mapping电信号研究

【摘要】:背景——房颤(Atrial fibrillation, AF),被认为有着复杂的起因和维持机制。对于复率后的窦性心律信号,如何评估长时间维持房颤的可能性未见有过报道。论文的目的是通过分析快速犬类房颤模型中阶梯状乙酰胆碱(Acetylcholine, ACh)剂量下的窦性心律电信号,探索研究心房易颤性(诱发出房颤的可能性)的方法。 方法和结果——实验建立了8只犬快速房颤模型,在动物的左右心房、左右心房后部及4条肺静脉根部放置了128路电极构成的电标记系统。实验中注入阶梯式浓度的ACh,得到对应不同ACh浓度的窦性心电。通过对犬采集CT断层图片,利用GVF-Snake模型采用基于轮廓线的三维重建技术建立了动物体的三维模型,并通过反向距离加权平均和Delaunay三角剖分的方法实现了模型的参数显示。在右心房表面电极施力(?)bust刺激,记录刺激出的房颤时间和次数,通过房颤总维持时间来表征不同ACh浓度下房颤维持的可能性。并尝试找到可以与房颤维持时间对应的某种窦性心电的参数。根据相同步的概念,提出了心房工作在与窦房结同步的工作模式下的假设,利用相位同步的方法,计算所有导联对窦房结的窦性心电相位差矩阵香农熵(SE),并发现SE对ACh剂量曲线与房颤总时间对ACh曲线相似。我们随后也测试了信号频率及导联数目对SE曲线的影响。 结论——我们发现窦性心电相位差矩阵的香农熵SE的升高意味着诱发出的房颤维持长时间的可能性升高。为复律后通过窦性心电判断房颤复发可能性提供了一种方法。
[Abstract]:Background-Atrial fibrillation (Atrial fibrillation, AF) is considered to have complex causes and maintenance mechanisms. How to evaluate the possibility of long-term maintenance of atrial fibrillation has not been reported for sinus rhythm signals after recovery. The purpose of this paper is to study the possibility of atrial fibrillation (AF) by analyzing the sinusoidal electrical signals at the dose of acetylcholine (ach) in a fast canine atrial fibrillation model. Methods and results: eight canine models of rapid atrial fibrillation were established. An electrical labeling system composed of 128 electrodes was placed in the left and right atrium, the posterior part of the left and right atrium and the root of 4 pulmonary veins. In the experiment, step by step concentration of ache was injected and sinus electrocardiogram corresponding to different ach concentration was obtained. By collecting CT images of dogs and using GVF-Snake model, the 3D models of animals and animals are established by using the three-dimensional reconstruction technique based on contour, and the parameter display of the model is realized by means of inverse distance weighted average and Delaunay triangulation. The time and times of atrial fibrillation induced by bust stimulation at right atrial surface electrode were recorded, and the possibility of atrial fibrillation maintenance under different ach concentrations was characterized by the total duration of atrial fibrillation. And try to find some sinus parameters corresponding to the duration of atrial fibrillation. Based on the concept of phase synchronization, the assumption that atrial work is in synchronous mode with sinus node is proposed, and the method of phase synchronization is used. The phase difference matrix Shannon entropy (SE) of sinus electrocardiogram for all leads to sinoatrial node was calculated, and it was found that the dose curve of SE to ach was similar to that of total time of atrial fibrillation. We also tested the influence of signal frequency and lead number on SE curve. Conclusion-We found that the increase of Shannon entropy SE of the phase difference matrix of sinus electrocardiogram means that the induced atrial fibrillation is more likely to last for a long time. It provides a method for judging the possibility of recurrence of atrial fibrillation by sinus electrocardiogram after cardioversion.
