布地奈德福莫特罗粉吸入剂联合中医药辨证治疗老年咳嗽变异性哮喘

【摘要】：目的考察布地奈德福莫特罗粉吸入剂联合中医药辨证治疗老年咳嗽变异性哮喘(CVA)的临床疗效。方法将2014年7月至2016年8月青海省中医院收治的85例老年CVA患者分为观察组(43例)与对照组(42例),对照组采用布地奈德福莫特罗粉吸入剂吸入治疗,观察组在对照组基础上立足于将祛风作为首要,同时加用中药分夹寒、夹热、夹燥、夹痰等4种证型行辨证治疗,两组患者疗程均为14d。比较两组患者的肺功能指标[包括第1秒用力呼气体积(FEV_1)、FEV_1与用力肺活量(FVC)比值(FEV_1/FVC)及最大呼气流量(PEF)],血清指标[包括白细胞介素-4/γ-干扰素(IL-4/IFN-γ)比值与IL-17]及临床疗效。结果治疗后,两组FEV_1、FEV_1/FVC、PEF均较治疗前增大,血清IL-4/IFN-γ比值与IL-17水平均较治疗前降低,差异均有统计学意义(P0.05);且治疗后观察组FEV_1、FEV_1/FVC均高于对照组(t=2.549、2.060,P=0.008、0.020),血清IL-4/IFN-γ比值与IL-17水平均低于对照组(t=16.229、4.417,P0.001);观察组的愈显率与总有效率分别为76.74%与95.35%,对照组分别为52.38%与78.57%,组间比较差异均有统计学意义(χ~2=5.522、5.308,P=0.027、0.033)。结论较单一使用布地奈德福莫特罗粉吸入剂,联合中医药辨证治疗老年CVA更具优势。
[Abstract]:Objective to investigate the clinical effect of budesonide formoterol powder inhalation combined with traditional Chinese medicine (TCM) in the treatment of senile cough variant asthma (CVA).Methods from July 2014 to August 2016, 85 elderly patients with CVA were divided into observation group (n = 43) and control group (n = 42). The control group was treated with inhaled budesonide formoterol powder.Observation group based on the control group on the basis of dispelling wind as the first, at the same time with traditional Chinese medicine divided into cold, heat, dryness, phlegm, and other four syndrome differentiation treatment, the two groups of patients were treated for 14 days.The pulmonary function indexes (including FEV1 / FEV1 / FEV1 / FVC-1 / FEV1 / FVC-1) and peak expiratory flow (PEFF) were compared between the two groups. The serum indexes [including interleukin-4 / 纬 -interferon IL-4 / IL-4 / IFN- 纬 ratio and IL-17] and clinical efficacy were compared.Results after treatment, the FEV _ 1 / FEV _ 1 / FEV _ 1 / FEV _ 1 / FVCV / PEF in both groups increased, and the ratio of IL-4 / IFN- 纬 and the level of IL-17 in serum decreased compared with those before treatment.The difference was statistically significant (蠂 ~ (2 +) 5.522 ~ (5.308)) (P = 0.027) 0.033 (P < 0.05).Conclusion compared with budesonide formoterol powder inhalation alone and combined with traditional Chinese medicine, the treatment of senile CVA is more advantageous.
