呼吸肌锻炼联合无创正压通气在稳定期重度慢性阻塞性肺疾病患者中的应用

【摘要】:目的探讨呼吸肌锻练联合无创通气的肺康复策略的临床应用价值。方法将2012年1月至2015年12月在广州呼吸疾病研究所诊断明确的稳定期慢性阻塞性肺疾病(慢阻肺)患者88例随机分为联合组、呼吸肌锻炼组、无创组及氧疗组并干预8周。评价生活质量(SRI)、呼吸肌力(MIP、MEP)、呼吸困难(MRC)、运动耐量(6MWT)及肺功能。结果联合组、锻炼组及无创组SRI总分、6MWT、MRC均显著改变(P均0.05),且联合组△SRI、△6MWD、△MRC和锻炼组、无创组间差异有统计学意义(P均0.05)。联合组、锻炼组MIP、MEP显著性提高(P均0.05),且联合组△MIP、△MEP与锻炼组间差异有统计学意义(P0.05)。肺功能差异无统计学意义(P均0.05)。结论呼吸肌锻练联合无创通气康复策略临床效果最优。
[Abstract]:Objective to explore the clinical application value of pulmonary rehabilitation strategy of respiratory muscle training combined with noninvasive ventilation. Methods 88 patients with chronic obstructive pulmonary disease (COPD) diagnosed in Guangzhou respiratory disease institute from January 2012 to December 2015 were randomly divided into combined group, respiratory muscle exercise group, non invasive group and oxygen therapy group and intervention for 8 weeks. The quality of life (SRI), respiratory muscle strength (MIP, MEP), dyspnea (MRC), exercise tolerance (6MWT) and lung function. Results combined group, exercise group and non invasive group SRI total score, 6MWT, MRC were significantly changed (P 0.05), and the joint group Delta SRI, Delta 6MWD, Delta MRC and exercise groups were statistically significant (all 0.05). The joint group, exercise group, significant There was a significant difference in sex (P 0.05), and the difference between the joint group Delta MIP, Delta MEP and the exercise group was statistically significant (P0.05). There was no significant difference in pulmonary function (P 0.05). Conclusion the clinical effect of respiratory muscle training combined with non invasive ventilation was the best.
