肺癌患者姑息治疗后肺部感染病原菌分布与耐药性分析

【摘要】:目的了解肺癌患者姑息治疗后肺部感染的病原菌分布及耐药性,为医院合理用药提供参考依据。方法选择2014年6月-2015年7月于医院肿瘤内科接受姑息治疗过程中发生肺部感染的肺癌患者106例,收集患者痰液标本,进行细菌培养、鉴别及药敏试验,对病原菌的分布特点、类型及耐药性进行分析,数据采用SPSS 17.0软件进行统计分析。结果 106份标本中培养出病原菌174株,以革兰阴性菌为主,共102株占58.62%;铜绿假单胞菌、肺炎克雷伯菌及大肠埃希菌对亚胺培南的耐药率低,均20.00%;主要革兰阳性菌对万古霉素的耐药率,均为0;主要真菌对酮康唑的耐药率低,均为0。结论肺癌姑息治疗合并肺部感染患者的病原菌以革兰阴性菌为主,临床应加强对革兰阴性菌及其耐药性检测,在对症治疗的基础上,需要建立完善的消毒灭菌机制,培养医护人员的无菌操作意识,合理使用抗菌药物,减少耐药菌的产生。
[Abstract]:Objective to investigate the distribution of pathogenic bacteria and drug resistance of pulmonary infection after palliative treatment in patients with lung cancer, and to provide reference for rational use of drugs in hospitals. Methods 106 lung cancer patients who received palliative treatment from June 2014 to July 2015 were selected. The sputum samples were collected, bacteria culture, identification and drug sensitivity test were performed to analyze the distribution of pathogenic bacteria. Types and drug resistance were analyzed, and the data were analyzed by SPSS 17.0 software. Results 174 strains of pathogenic bacteria were isolated from 106 samples, 102 strains were Gram-negative bacteria, accounting for 58.62.The resistance rate of Pseudomonas aeruginosa, Klebsiella pneumoniae and Escherichia coli to imipenem was low. The resistance rate of main gram-positive bacteria to vancomycin was 0, and the resistance rate of major fungi to ketoconazole was 0. Conclusion Gram-negative bacteria are the main pathogenic bacteria in patients with pulmonary infection after palliative treatment of lung cancer. The detection of Gram-negative bacteria and their drug resistance should be strengthened in clinic. On the basis of symptomatic treatment, it is necessary to establish a complete disinfection and sterilization mechanism. Cultivate the aseptic operation consciousness of medical staff, use antibiotics rationally and reduce the production of drug-resistant bacteria.
