PCT、SAA测定在西宁地区老年AECOPD患者中的临床意义

【摘要】:目的:在我国,慢性阻塞性肺疾病(COPD)为主要呼吸系统疾病之一。气道炎症引起急性加重(AECOPD),加剧病情,使肺功能逐渐下降,这是其重要的发展过程。故优选炎症指标,对评估肺功能及病情估测有重要的指导作用。选取PCT、SAA两种炎症指标,通过检测两者在西宁地区老年AECOPD患者中血清含量,及治疗后稳定期和健康者中含量,探究PCT、SAA变化与患者肺功能的相互关系,为AECOPD患者的临床诊断和治疗提供新的方法。方法:研究对象在参与者知情同意情况下,遴选青大附属医院呼吸内科2014年12月 2015年12月入院的AECOPD患者60例(年龄≥60岁)。所有患者诊断按照2015年慢性阻塞性肺疾病全球防治创议(GOLD 2015),设立急性加重期组(治疗前),以及治疗后的稳定期组,以体检中心15例体检健康者作为对照组。依照GOLD2015肺功能具体分级,将AECOPD患者分为A组(轻度)、B组(中度)、C组(重度和极重度),测定各组PCT、SAA,以此探讨两指标和COPD的相关联系。结果:1.PCT、SAA指标:(1)急性加重期血清PCT、SAA含量均高于稳定期及健康对照组(P0.05);(2)稳定期PCT、SAA含量高于健康对照组(P0.05);(3)急性加重期C组PCT含量与A、B组相比均有升高(P0.05),B组PCT含量与A组相互比较有显著升高(P0.05)。A组、B组及C组之间SAA含量无明显统计学差异。2.PCT、SAA指标与肺功能指标关系:(1)急性加重期PCT含量与FEV1/pre%指标呈显著负相关(r= 0.77,P0.05),与FEV1/FVC%指标呈显著负相关(r= 0.56,P0.05);(2)急性加重期SAA含量与FEV1/pre%指标无明显相关(r= 0.33,P0.05),与FEV1/FVC%指标无明显相关(r= 0.09,P0.05)。结论:1.PCT、SAA含量可反映西宁地区老年COPD患者发生急性加重变化;2.PCT、SAA含量的测定可用于反映AECOPD患者的预后;3.PCT含量也可用于评估AECOPD患者病情,而SAA含量可以作为辅助指标之一。
