老年轻度认知障碍患者Stroop效应的诊断界值

【摘要】:目的探索老年轻度认知障碍(MCI)患者客观量化的临床筛检指标,简化测评手段,减少主观评判误差。方法采用1∶1病例对照研究方法确定MCI组和对照组各46例,应用Stroop色字干扰测验评定其认知功能缺损程度。结果 MCI与对照组Stroop效应耗时A、B、C、(C-B)/A和正确数A、B、C、(B-C)/A均有统计学差异(P0.05或P0.01)。Stroop效应耗时A、B、(C-B)/A与Mo CA总分、视空间、注意力、记忆力(耗时A除外)、抽象思维存在相关性,耗时C与Mo CA总分、注意力、记忆力存在相关性(P0.05或P0.01);正确数A与注意力存在相关性,正确数B与Mo CA总分、注意力、记忆力、命名存在相关性,正确数C与Mo CA总分、注意力存在相关性,正确数(B-C)/A与Mo CA总分、视空间、注意力、记忆力存在相关性(P0.05或P0.01)。Stroop效应受试者工作特征(ROC)曲线分析,耗时增加率(C-B)/A38.5%时筛检MCI的灵敏度71.7%、特异度71.6%、约登指数43.3%;正确数递减率(B-C)/A4.2%时,灵敏度73.9%、特异度78.3%、约登指数52.2%;耗时增加率38.5%或正确数递减率4.2%并联筛检MCI的灵敏度91.3%、特异度93.5%、约登指数84.8%。结论 MCI患者存在多重领域认知功能损害,主观测评难度较大,Stroop效应诊断MCI具有较好的灵敏度和特异度,测验简单快捷,较为适合作为老年人群体检中筛查MCI的检测工具。
[Abstract]:Objective to explore the objective and quantitative clinical screening indexes of elderly patients with mild cognitive impairment (MCI), simplify the evaluation method and reduce the subjective judgment error. Methods the 1:1 case-control study was used to determine 46 cases in MCI group and 46 cases in control group.
