静息态fMRI在无痴呆型血管性认知障碍患者基线脑活动变化中的初步研究

【摘要】:目的:利用基于低频振幅算法(ALFF)的静息功能磁共振(fMRI)技术探讨无痴呆型血管性认知障碍(VCIND)与正常老年人静息状态下基线脑活动变化。 方法:22名VCIND患者作为研究对象,23名正常老年人(NC)作为对照组,采用低频振幅(the amplitude of low-frequency fluctuations,ALFF)分析方法,比较两组平均脑ALFF图的差异;提取两组被试双侧后扣带回/楔前叶(PCC/Pcu)的ALFF值,检验VCIND组与正常对照组之间是否有统计学差异,并将VCIND组PCC/PCu的ALFF值与MoCA评分进行相关分析。 结果:1.VCIND患者在静息状态下较对照组在以下脑区ALFF值增高,包括左额下回/左颞上回(Z=-4.0682)、右额下回/右颞上回(Z=-3.9720)、左侧岛叶(Z=-3.5773);VCIND患者组在以下脑区出现ALFF值减低:舌回/小脑后叶(Z=3.8921)、前扣带皮层(Z=3.8081)、右楔叶/右舌回(Z=4.8272)、后扣带回/楔前叶(Z=3.9741)(P0.05);2.两组被试PCC/Pcu的ALFF值分别为:VCIND组0.9861±0.0192,,正常对照组0.9984±0.0207,正常对照组较VCIND组ALFF值更高(两样本t检验t=2.063,P=0.0450.05);3.VCIND组PCC/PCu的ALFF值与MoCA评分相关分析,相关系数R=0.455,方差分析F=5.22,P=0.033,可认为VCIND组患者PCC/PCu的ALFF值与MoCA评分呈正相关。 结论:VCIND患者在静息状态下ALFF有显著改变,主要表现为语言中枢等认知区域的局部ALFF增强及DMN等功能连接点的ALFF减低。
[Abstract]:Aim: to investigate the baseline brain activity changes of VCIND without dementia type vascular cognitive impairment (VCIND) and the rest state of normal elderly by using resting functional magnetic resonance (Mr) technique based on low frequency amplitude algorithm (ALFF). Methods Twenty two patients with VCIND were selected as control group. The low frequency amplitude of the amplitude of low-frequency structures (ALF) was used to compare the difference of average brain ALFF between the two groups. The ALFF values of the posterior cingulate gyrus / anterior cuneate lobe of the two groups were extracted to test whether there was a statistical difference between the VCIND group and the normal control group. The correlation between the ALFF value of the PCC/PCu and the MoCA score in the VCIND group was analyzed. Results 1. The ALFF values of VCIND patients in the rest state were higher than those in the control group in the following brain regions. In the left inferior frontal gyrus / left superior temporal gyrus Z-4.0682A, right inferior frontal gyrus / right superior temporal gyrus Z-3.9720, the left insular gyrus Z-3.5773V VCIND patients had decreased ALFF values in the following areas: lingual gyrus / posterior lobe Z3.8921, anterior cingulate cortex Z3.8081, right cuneate / right lingual gyrus Z4.8272, posterior cingulate gyrus / cuneate gyrus / posterior cingulate gyrus / cuneate gyrus. The ALFF value of PCC/Pcu in the two groups was 0.9861 卤0.01922 in the control group and 0.9984 卤0.0207 in the normal control group. The ALFF value in the normal control group was higher than that in the VCIND group. The correlation coefficient was 0.455, and the variance analysis of variance was 5.22%. It was suggested that the ALFF value of PCC/PCu was positively correlated with the MoCA score in the VCIND group. Conclusion there were significant changes in ALFF in patients with VCIND at rest, mainly manifested by the enhancement of local ALFF in cognitive regions such as language center and the decrease of ALFF in functional junctions such as DMN.
