酒精成瘾者静息态默认网络fMRI研究

【摘要】:目的:应用静息态功能磁共振成像的独立成分分析及种子相关分析方法,研究酒精成瘾者默认网络改变及其内异常脑区功能连接的改变,以及功能连接改变与临床变量的关系,探讨酒精成瘾的可能神经机制。 方法:采用3.0T磁共振对30例酒精成瘾者与30例性别、年龄、受教育程度及尼古丁依赖程度匹配健康志愿者进行静息态功能磁共振扫描,采取独立成分分析方法提取默认网络,对两组默认网络进行统计分析得到默认网络对比差异区域;选取差异区域中的一重要区域作为种子点进行功能连接分析,且将功能连接强度与临床量表进行Pearson相关分析。 结果:相对于对照组,酒精成瘾组默认网络在右楔前叶、左小脑脚Ⅱ区功能连接减低,而在左后扣带回、右颞中回、左颞上回及右楔前叶区域功能连接增强;左小脑脚Ⅱ区与双侧楔前叶、右内侧额叶、左额上回之间的功能连接减弱,而与左小脑半球Ⅵ区、双侧脑岛、双侧顶下小叶之间的功能连接增强,,且左小脑脚Ⅱ区与双侧脑岛功能连接强度与饮酒时间呈正相关。 结论:酒精成瘾者默认网络功能异常,且默认网络内左小脑脚Ⅱ区参与的神经环路异常,结合相关分析结果,提示静息态功能磁共振成像对于酒精成瘾复杂神经机制的揭示及临床评估具有一定的价值。
[Abstract]:Objective: to study the changes of default network and functional connections in abnormal brain regions of alcohol addicts and the relationship between the changes of functional connections and clinical variables by means of independent component analysis and seed correlation analysis of resting functional magnetic resonance imaging (fMRI).To explore the possible neural mechanism of alcohol addiction.Methods: 30 patients with alcohol addiction and 30 healthy volunteers with sex, age, education and nicotine dependence were examined with 3.0T magnetic resonance imaging (MRI), and the default network was extracted by independent component analysis (ICA).Through the statistical analysis of two groups of default networks, the contrast area of the default network is obtained, and an important area in the difference area is selected as the seed point for functional connection analysis, and the Pearson correlation analysis between the strength of functional connection and the clinical scale is carried out.Results: compared with the control group, the default network in the alcohol addiction group was decreased in the anterior lobe of right wedge, and the functional connections in the left posterior cingulate gyrus, the right middle temporal gyrus, the left superior temporal gyrus and the right anterior cuneate lobe were enhanced.The functional connections between the left cerebellum 鈪
