~1H-MRS在广泛性焦虑诊断中的应用

【摘要】:目的:探讨氢质子磁共振波谱(~1H-MRS)对广泛性焦虑诊断的价值。方法:搜集临床上确诊为广泛性焦虑的患者24例,健康志愿者24例,行常规MRI平扫及~1H-MRS扫描。测定双侧海马、双侧前扣带回、双侧后扣带回、双侧额中回皮质、双侧楔叶皮质、双侧额叶白质、双侧枕叶白质区的N-乙酰天门冬氨酸(NAA)、胆碱化合物(Cho)、肌酸(Cr)、肌醇(Mi)、丙氨酸、葡萄糖峰下面积均值,计算NAA/Cr、Cho/Cr、Mi/Cr、NAA/(Cho+Cr)、Cho/NAA、Lac/Cr值。结果:病例组双侧海马Cho/Cr、Mi/Cr、Cho/NAA较对照组明显升高,,左侧前扣带回Cho/Cr及Lac/Cr较对照组升高,双侧额中回皮质Lac/Cr及葡萄糖较对照组升高;右侧后扣带回MI/Cr较对照组升高;左侧楔叶皮质NAA/(Cho+Cr)、Cho/NAA及Lac/Cr较对照组升高,右侧楔叶皮质NAA/Cr、Cho/Cr、NAA/(Cho+Cr)、Cho/NAA及葡萄糖较对照组升高;左侧楔叶白质Lac/Cr与对照组相比升高,右侧楔叶白质的Lac/Cr及葡萄糖较对照组升高;病例组组内比较,左侧额叶白质NAA/Cr、NAA/(Cho+Cr)较右侧升高,差异有统计学意义(p㩳0.05)。结论:广泛性焦虑患者在常规脑MRI平扫阴性时,~1H-MRS可以检测到脑代谢异常,为焦虑症的定量诊断提供了一定的诊断依据。
[Abstract]:Objective: to evaluate the value of 1H-MRS in the diagnosis of generalized anxiety. Methods: 24 patients with generalized anxiety and 24 healthy volunteers were examined by conventional MRI scan and 1H-MRS scan. Bilateral hippocampal, bilateral anterior cingulate gyrus, bilateral posterior cingulate gyrus, bilateral middle frontal gyrus cortex, bilateral cuneate cortex, bilateral frontal white matter, bilateral occipital white matter area N- acetyl aspartate NAA, choline compound Chochus, creatine cingulate, inositol Mii, alanine, were measured in bilateral hippocampus, bilateral anterior cingulate gyrus, bilateral cuneiform cortex, bilateral frontal white matter, bilateral occipital white matter area. The average area under the peak of glucose is calculated by calculating the value of NAA / CrCho-Cr / Cr / Cr / NAA / NAA / Cr = Cho / Cr / NAA / AC / Cr value. Results: in the bilateral hippocampus, the Cho/Cr and Lac/Cr of left anterior cingulate gyrus were significantly higher than those in control group, the Lac/Cr and glucose in bilateral middle frontal gyrus were higher than those in control group, and the MI/Cr in right posterior cingulate gyrus was higher than that in control group. The NAA/(Cho / NAA and Lac/Cr in the left cuneiform cortex were higher than those in the control group, and the NAA / NAA and glucose in the right cuneiform cortex were higher than those in the control group, while the Lac/Cr in the left cuneiform white matter was higher than that in the control group, and the Lac/Cr and glucose in the right cuneiform white matter were higher than those in the control group. Compared with the right side, the NAA / CRN NAA / Cho Cr in the left frontal white matter was higher than that in the right side, and the difference was statistically significant (P < 0.05). Conclusion: 1H-MRS can detect abnormal brain metabolism in patients with generalized anxiety when conventional MRI scan is negative, which provides a diagnostic basis for quantitative diagnosis of anxiety disorder.
