文拉法辛缓释片对广泛性焦虑患者心率变异性的影响

【摘要】：目的比较文拉法辛缓释片与盐酸帕罗西汀对广泛性焦虑(GAD)患者心率变异性(HRV)的影响。方法选取80例GAD患者随机分为文拉法辛缓释片组和盐酸帕罗西汀组。文拉法辛缓释片组患者口服文拉法辛缓释片,初始剂量75 mg/d,最大剂量不超过225 mg/d,1次/d。盐酸帕罗西汀组患者口服盐酸帕罗西汀,初始剂量20 mg/d,最大剂量不超过60 mg/d,早餐时顿服。两组患者疗程均为6周,分别于治疗前、治疗6周后进行24 h HRV监测和汉密顿焦虑量表(HAMA)评分,以HAMA总分减分率作为疗效评定。结果治疗后两组HRV指标均较治疗前升高(P0.05),文拉法辛缓释片组与盐酸帕罗西汀组比较,全部窦性心搏R-R间期标准差及高频升高,低频/高频下降(P0.05)。治疗6周后两组HAMA总分均较治疗前下降(P0.05)。结论文拉法辛缓释片与盐酸帕罗西汀均能提高广泛性焦虑患者HRV,文拉法辛缓释片对自主神经功能的改善作用可能优于盐酸帕罗西汀。
[Abstract]:Objective to compare the effects of venlafaxine sustained-release tablets and paroxetine hydrochloride on heart rate variability (HRV) in patients with generalized anxiety (GAD). Methods 80 patients with GAD were randomly divided into two groups: venlafaxine sustained-release tablet group and paroxetine hydrochloride group. The patients of venlafaxine sustained release tablet group were given venlafaxine sustained-release tablets, the initial dose of 75 mg/d, maximum dose was not more than 225 mg/d,1 per day. Patients in paroxetine hydrochloride group were given paroxetine hydrochloride orally. The maximum dose of paroxetine hydrochloride for 20 mg/d, was not more than 60 mg/d, for breakfast. The course of treatment in both groups was 6 weeks. 24 h HRV monitoring and Hamilton anxiety scale (HAMA) score were performed before and after 6 weeks of treatment respectively. The total HAMA score reduction rate was taken as the curative effect. Results compared with paroxetine hydrochloride group, the standard deviation and high frequency of R-R interval of all sinus beats increased and low frequency / high frequency decreased in venlafaxine sustained-release tablet group and paroxetine hydrochloride group (P0.05). After 6 weeks of treatment, the total score of HAMA in both groups was lower than that before treatment (P0.05). Conclusion both venlafaxine sustained-release tablets and paroxetine hydrochloride can improve the autonomic nervous function of patients with generalized anxiety.
