利培酮合用文拉法辛对精神分裂症患者阴性症状及社会功能改善的对照研究

【摘要】：目的探讨利培酮合用抗抑郁药文拉法辛对精神分裂症阴性症状、社会功能恢复的疗效。研究方法本研究采用随机分组的方法,将符合CCMD-3精神分裂症诊断标准的80例患者随机分为利培酮±文拉法辛组(研究组)和利培酮组(对照组)各40例。其中研究组完成35例,对照组完成36例。治疗时间为12周,分别于治疗前、治疗4周、8周、12周末使用阳性与阴性症状量表(the Positive and Negative Syndrome Scale, PANSS)精神病患者社会功能评定量表(Personal and Social Performance, PSP)评估临床疗效,锥体外系反应量表(A Rating Scale for Extrapyramidal Side Effects, RSESE)评估治疗副反应。并于治疗前、治疗4周、8周、12周末行血常规、血生化及心电图检查来评估药物的安全性结果 1.两组组内比较：与治疗前相比,研究组与对照组在4周末、8周末、12周末PANSS总分、阳性因子分、阴性因子分、一般精神病理症状总分均有显著下降(P0.05或P0.01)；两组间比较：研究组与对照组在第12周末PANSS,总分、阴性因子分、一般精神病理症状总分差异均有统计学意义(P0.05或P0.01)。 2.两组组内比较：与治疗前相比,研究组与对照组在4周末、8周末、12周末PSP总分均有显著下降(P0.01)；两组间比较：研究组与对照组在第12周末PSP总分差异有统计学意义(P0.05)。 3.不良反应的发生率两组差异无统计学意义(P0.05),不良反应多为轻度,最常见不良反应均为锥体外系反应。结论 1.利培酮合用文拉法辛可以改善精神分裂症患者的阴性症状； 2.利培酮合用文拉法辛可以改善精神分裂症患者社会功能； 3.利培酮合用文拉法辛治疗精神分裂症的安全性较好,具有良好的临床实用性。
[Abstract]:Objective to investigate the efficacy of risperidone combined with venlafaxine in the recovery of social function and negative symptoms of schizophrenia. Methods 80 patients with CCMD-3 schizophrenia were randomly divided into risperidone 卤venlafaxine group (study group) and risperidone group (control group). 35 cases were completed in the study group and 36 cases in the control group. The treatment time was 12 weeks. Before treatment, 4 weeks, 8 weeks and 12 weeks of treatment, (Personal and Social Performance, PSP) was used to assess the clinical efficacy of (the Positive and Negative Syndrome Scale, PANSS) psychiatric patients with positive and negative symptoms. Treatment side effects were evaluated by extrapyramidal response scale (A Rating Scale for Extrapyramidal Side Effects, RSESE). Before treatment, 4 weeks, 8 weeks and 12 weeks of treatment, blood routine, blood biochemistry and electrocardiogram were used to evaluate the safety of the drug. 1. Comparison between the two groups: compared with before treatment, the total scores of PANSS, positive factor, negative factor and general psychopathological symptoms in the study group and the control group decreased significantly at the 4th, 8th and 12th weekend (P0.05 or P0.01). Comparison between the two groups: the total scores of PANSS, negative factor and general psychopathological symptoms were significantly different between the study group and the control group at the end of the 12th week (P0.05 or P0.01). 2. Comparison between the two groups: compared with before treatment, the total scores of PSP in the study group and the control group decreased significantly at the end of 4, 8 and 12 weeks (P0.01); Comparison between the two groups: the study group and the control group in the 12th week PSP total score difference was statistically significant (P0.05). 3. There was no significant difference in the incidence of adverse reactions between the two groups (P0.05). The most common adverse reactions were extrapyramidal reactions. Conclusion 1. Risperidone combined with venlafaxine can improve the negative symptoms of schizophrenia; 2. Risperidone combined with venlafaxine can improve the social function of schizophrenic patients. Risperidone combined with venlafaxine is safe and practical in the treatment of schizophrenia.
