酒精诱导血小板凋亡

【摘要】:【背景】酒精饮品在日常生活中最流行的饮品之一。然而,酗酒已成为世界各国共同的社会问题。酒精滥用导致严重的临床事件的发生,如血小板减少,出血,但发病机制尚不完全清楚。【方法】收集18-25岁、两周内未服用任何药物的健康志愿者的空腹新鲜静脉全血,分离得到洗涤血小板及富含血小板血浆(Platelet rich plasma,PRP)。洗涤血小板(3×108/ml)与不同浓度酒精或者阴性对照二甲基亚砜(Dimethyl sulfoxide,DMSO)于37℃孵育2小时,使用流式细胞仪检测血小板线粒体跨膜电位(Mitochondrial inner transmembrane potential,ΔΨm)变化、血小板活化指标P-选择素(P-Selectin)及血小板激活复合物(Patelet activated complex-1,PAC-1)的结合率。使用Western blot检测酒精与正常人洗涤血小板孵育后血小板促凋亡蛋白Bax,抗凋亡蛋白Bcl-2的表达和caspase-3的活化情况。为了探明c-Jun氨基末端激酶(c-jun NH2-terminal kinase,JNK)蛋白家族在酒精诱导的血小板凋亡过程中的作用,我们使用Western blot检测JNK蛋白家族磷酸化表达水平的变化。同时我们研究了酒精对不同致聚剂诱导血小板聚集功能的影响,探讨酒精对血小板功能有无损伤,将PRP与酒精(200 mg/dl)或者阴性对照(DMSO)于37℃孵育2小时,洗涤血小板与酒精(200 mg/dl)或阴性对照(DMSO)于37℃孵育2小时后,PRP中加入二磷酸腺苷(Adenosine diphosphate,ADP,10μmol/L),洗涤血小板中加入凝血酶(Thrombin,0.5 U/m L)或者胶原(collagen,5μg/m L),1000 rpm剪切力的情况下,使用血小板聚集仪检测血小板聚集程度。在体内实验中,对C57BL/6J小鼠进行灌胃,之后使用Sysmex KX-21N血细胞计数仪测定小鼠血小板计数,检测尾出血时间以及胃粘膜出血情况。【结果】乙醇呈剂量依赖性诱导线粒体内跨膜电位的去极化,上调Bax蛋白的表达量和Bcl-2的表达量的下调,和caspase-3的活化。乙醇不诱导P-选择素或PAC-1的表达,并明显降低胶原,凝血酶,和ADP诱导的血小板聚集。此外,酒精诱导JNK凋亡通路的激活。在急性酒精中毒的体内小鼠模型中,乙醇明显降低循环血小板数,延长尾出血时间,并造成胃粘膜出血。【结论】这些数据表明,乙醇导致线粒体介导的血小板的细胞凋亡,导致循环血小板数量的减少和体内止血机制损害。这些发现揭示急性酒精中毒患者出血现象的可能发病机制。
