应用fMRI探讨海洛因成瘾者后扣带回相关功能变化

【摘要】:目的采取线索反应模式及静息态功能连接的方法,利用功能磁共振成像(fMRI)探索与海洛因成瘾者复吸的脑区功能定位,确定与环境因素导致复吸的相关脑区,重点了解后扣带回在海洛因成瘾中的功能作用。 方法海洛因成瘾者和对照者各15例,使用3.0T Philips Power Station MR成像及功能磁共振视频刺激系统:(1)采用线索诱发模式,使用E-Prime编辑海洛因使用视频,在观看海洛因相关视频同步进行功能磁共振扫描,利用事件相关功能磁共振及使用独立成分分析的方法,了解海洛因成瘾者在海洛因相关场景刺激下,后扣带回及相关脑区的激活与正常被试的差异;(2)采用静息态模式,利用功能连接的方法,以后扣带回为感兴趣区,分别与全脑进行功能连接分析,了解海洛因成瘾者在静息状态下,后扣带回功能连接情况及与正常被试的差异。 结果(1)海洛因成瘾组与正常对照组相比,在相关线索的刺激下,双侧前额叶,左前扣带回,,双侧后扣带回,左侧顶叶,左侧颞下回的激活显著降低;双侧伏核、右侧海马以及枕叶的激活明显增强;(2)当以左侧后扣带回作为感兴趣区做全脑的功能连接时发现,相比较与对照组,海洛因成瘾组左侧后扣带回与右侧颞叶的功能连接增强,与右侧额叶、丘脑以及左侧顶叶的功能连接明显降低;选取右侧后扣带回作为感兴趣区进行全脑功能连接时,海洛因成瘾组右侧后扣带回与左侧额叶、左侧顶叶以及左侧丘脑的功能连接明显低于正常对照组(P<0.05); 结论(1)海洛因成瘾者,在线索诱发及静息状态下,后扣带回功能均出现异常,后扣带回参与了海洛因成瘾者奖赏系统的组成,与海洛因成瘾的产生,维持,复吸密切相关。(2)在线索诱发情况下,海洛因成瘾者奖赏系统及相关脑区明显激活,奖赏系统等相关脑区共同参与了场景诱导的海洛因复吸。
[Abstract]:Objective to explore the functional localization of brain regions in heroin addicts with functional magnetic resonance imaging (fMRI) using the methods of cue response pattern and resting functional connection, so as to determine the brain regions related to environmental factors leading to relapse, and to explore the functional localization of brain regions associated with relapse of heroin addicts by functional magnetic resonance imaging (fMRI). To understand the function of posterior cingulate gyrus in heroin addiction. Methods heroin addicts (n = 15) and control group (n = 15) were treated with 3.0T Philips Power Station MR imaging and functional magnetic resonance video stimulation system. (1) lead-induced mode was used to edit heroin use video using E-Prime. After watching heroin-related videos synchronously performing functional magnetic resonance scans, using event-related functional magnetic resonance imaging and independent component analysis, we found out that heroin addicts were stimulated by heroin-related scenes. The activation of posterior cingulate gyrus and related brain areas was different from that of normal subjects. (2) using the static mode and the functional connection method, the cingulate gyrus is the region of interest, and the whole brain is analyzed to understand the heroin addict in the resting state, and then the cingulate gyrus is the region of interest, and the whole brain is used to analyze the functional connection of the heroin addict. Functional connection of posterior cingulate gyrus and its difference with normal subjects. Results (1) compared with the normal control group, the activation of bilateral frontal lobe, left anterior cingulate gyrus, bilateral posterior cingulate gyrus, left parietal lobe and left inferior temporal gyrus decreased significantly in heroin addiction group. The activation of bilateral nucleus accumbens, right hippocampus and occipital lobe was significantly increased. (2) when the left posterior cingulate gyrus was used as the functional connection of the whole brain, compared with the control group, the functional connection between the left posterior cingulate gyrus and the right temporal lobe and the right frontal lobe was enhanced in the heroin addicted group. The functional connection of thalamus and left parietal lobe decreased significantly. The functional connection between right posterior cingulate gyrus and left frontal lobe, left parietal lobe and left thalamus in heroin addicted group was significantly lower than that in normal control group (P < 0.05) when the right posterior cingulate gyrus was selected as the whole brain functional connection of the region of interest (P < 0.05). Conclusion (1) the function of the posterior cingulate gyrus is abnormal in both the cue-induced and resting state. The posterior cingulate gyrus is involved in the composition of the reward system of heroin addicts, and it is associated with the production and maintenance of heroin addiction, and that the posterior cingulate gyrus is involved in the formation and maintenance of heroin addiction. Relapse is closely related. (2) in the case of cue-induced heroin addicts, the reward system and the related brain regions are obviously activated, and the reward system and other related brain regions participate in the scene-induced heroin relapse.
