分裂型人格障碍青年男性睾酮水平与心理旋转表现之间的相关性:一项功能核磁共振影像学研究

【摘要】:背景:性激素在健康人群的视空间认知能力中具有重要意义,特别是睾酮被发现与视空间认知能力密切相关。已经有研究者尝试揭示精神分裂症患者的睾酮水平与心理旋转任务期间脑激活的关系,并且报告了精神分裂症男性患者在心理旋转表现上的缺陷与脑功能活动的紊乱。分裂型人格障碍(Schizotypal PersonalityDisorder, SPD)是通过与精神分裂症的遗传学关系而定义的一种人格障碍。本次研究以分裂型人格障碍者为对象,期待可以进一步认识分裂型人格障碍患者是否存在视空间能力的缺陷,试图揭示睾酮对精神分裂症谱系障碍的视空间能力影响的神经基质。 目标:1、检测分裂型人格障碍男性青年在视空间能力任务的表现,以及与视空间能力有关的脑区域的激活情况。2、试图探索睾酮与分裂型人格障碍男性青年的视空间能力之间的相关,并检验分裂型人格障碍男性青年的血清睾酮水平与心理旋转期间的脑区域激活之间的相关。 方法:17名分裂型人格障碍青年男性作为研究对象,以及17名健康青年男性作为对照组,参与了此次研究。抽取被试肘静脉血,用于血清睾酮水平的测量。对所有被试实施一个纸笔版的心理旋转能力测验(MRT-A, Peters et al.,1995),测量被试的视空间能力。采用3.0Tesla核磁共振扫描成像系统对所有被试进行扫描,使用E-prime软件呈现刺激图片(经典三维积木图形),旋转任务与非旋转条件交替呈现。使用SPM8软件对功能性核磁共振成像的数据进行处理,并对感兴趣区域进行相关分析。 结果:1、分裂型人格障碍组青年男性的血清睾酮水平显著高于健康对照组。2、在心理旋转测验的表现上,分裂型人格障碍组与对照组得分没有显著差异。3、心理旋转条件与非旋转条件相比较:SPD组在顶下小叶、楔前叶、额中回、额下回、额内侧回、小脑后叶、小脑前叶、丘脑的激活显著增多,,对照组在楔前叶、顶下小叶、额中回、额下回、小脑前叶、岛叶、丘脑激活显著增多。4、心理旋转任务的激活差异:与对照组相比,SPD组在额上回、额中回、额下回以及颞叶显示出更多的激活,然而与SPD组相比,对照组在颞叶、扣带回和岛叶表现出更多的激活。5、在SPD组,睾酮水平与额中回的激活表现出显著的正相关关系,而在对照组中,睾酮与脑激活之间没有表现出相关关系。 结论:本次实验结果进一步确认了心理旋转任务的激活模式。与健康青年男性的脑激活模式相比,分裂型人格障碍青年男性的脑激活模式表现出显著不同。分裂型人格障碍青年男性睾酮水平显著高于正常青年男性,并且其睾酮水平与额中回激活强度相关。这些结果说明了分裂型人格障碍青年男性在空间能力任务中脑功能活动的改变,并且睾酮可能在这些脑功能活动的变化中起到介导作用。
[Abstract]:Background: sex hormones play an important role in visual spatial cognitive ability of healthy people, especially testosterone is found to be closely related to visual spatial cognitive ability. Some researchers have tried to reveal the relationship between testosterone level and brain activation during the task of psychological rotation, and reported the defects of mental rotation and the disorder of brain function in male patients with schizophrenia. Mitotic personality disorder (Schizotypal PersonalityDisorder, SPD) is a personality disorder defined by its genetic relationship with schizophrenia. In this study, people with mitotic personality disorder are expected to further understand whether there is a defect in visual space ability in patients with mitotic personality disorder. This paper attempts to reveal the neural matrix of the effect of testosterone on the visual space ability of schizophrenia pedigree disorder. Objective: 1. To detect the performance of young men with split personality disorder in the task of visual space ability and the activation of brain areas related to visual space ability. 2, This paper attempts to explore the correlation between testosterone and visual space ability of young men with mitotic personality disorder, and to test the correlation between serum testosterone level and activation of brain region during psychological rotation. Methods: 17 young men with split personality disorder and 17 healthy young men as control group participated in the study. The blood of elbow vein was taken for the measurement of serum testosterone level. A paper-pen psychological rotation ability test (MRT-A, Peters et al.,1995) was performed on all subjects, and the visual space ability of the subjects was measured. 3.0Tesla nuclear magnetic resonance imaging system was used to scan all the subjects. E-prime software was used to present stimulus images (classical three-dimensional building block graphics). The rotation task and non-rotating conditions were presented alternately. SPM8 software was used to process the data of functional magnetic resonance imaging (fMRI), and the correlation analysis of the region of interest was carried out. Results: 1. The serum testosterone level of young men in the split personality disorder group was significantly higher than that in the healthy control group. 2. There was no significant difference in the scores between the split personality disorder group and the control group in the psychological rotation test. The activation of psychological rotation condition was significantly increased in subparietal lobule, anterior cuneiform lobe, middle frontalis gyrus, inferior frontalis gyrus, medial frontalis gyrus, posterior cerebellar lobe, anterior cerebellar lobe and thalamus in SPD group, while in control group, the activation of anterior cuneiform lobe and subparietal lobe was significantly higher than that in control group. Activation of the anterior cerebellar lobe, insular lobe and thalamus increased significantly in the middle forehead gyrus, inferior forehead gyrus, cerebellar anterior lobe, insular lobe and thalamus. 4. The activation difference of psychological rotation task: compared with the control group, the SPD group showed more activation in the superior forehead, middle forehead, inferior forehead and temporal lobe. However, compared with the SPD group, the control group showed more activation in the temporal lobe, cingulate gyrus and insular lobe. 5, in the SPD group, testosterone levels showed a significant positive correlation with the activation of the middle frontalis, while in the control group, There was no correlation between testosterone and brain activation. Conclusion: the results of this experiment further confirm the activation mode of psychological rotation task. Compared with the brain activation mode of healthy young men, the brain activation mode of young men with mitotic personality disorder was significantly different from that of healthy young men. The testosterone level of young men with mitotic personality disorder was significantly higher than that of normal young men, and its testosterone level was correlated with the activation intensity of middle forehead gyrus. These results suggest that young men with split personality disorder have changes in brain function during spatial ability tasks, and testosterone may play a mediating role in these changes in brain function.
