氟西汀与艾司西酞普兰对老年抑郁症外周血免疫T淋巴细胞亚群的影响

【摘要】:目的:考察氟西汀与艾司西酞普兰对老年抑郁症外周血免疫T淋巴细胞亚群的影响差异及疗效比较。方法:收集2010年4月-2013年6月就诊的原发性抑郁症患者89例,随机分为氟西汀组42例和艾司西酞普兰组47例,同期收集健康体检老年人49例作为对照。结果:与健康对照组比,2个组别的CD3、CD4和CD8降低(P0.05),CD4/CD8增高(P0.05),治疗前2个组别的外周血T淋巴细胞亚群无显著差异(P0.05),治疗后,2个组别的CD3、CD4和CD8升高(P0.05),CD4/CD8降低(P0.05),与氟西汀组比,艾司西酞普兰组的CD4/CD8显著降低(P0.05)。2个组别治疗前汉密尔顿抑郁量表(HAMD)评分无显著差异,2组治疗后各时段均下降明显,与治疗前比较有显著性差异(P0.05)。艾司西酞普兰组第1,2,4,8周的HAMD评分均低于氟西汀组,2组有显著性差异(P0.05)。2个组别的疗效和不良反应无显著差异(P0.05)。结论:艾司西酞普兰和氟西汀均可通过提高和调节T淋巴细胞的功能,以增强老年抑郁患者的免疫功能,艾司西酞普兰增加老年抑郁患者的免疫功能较氟西汀强。
[Abstract]:Objective: to investigate the effect of fluoxetine and escitalopram on peripheral blood immune T lymphocyte subsets in elderly patients with depression. Methods: 89 patients with primary depression from April 2010 to June 2013 were randomly divided into fluoxetine group (n = 42) and escitalopram group (n = 47). At the same time, 49 elderly patients were collected as control. Results: compared with the healthy control group, CD3,CD4 and CD8 decreased (P 0.05) and CD4/CD8 increased (P 0.05) in the two groups. There was no significant difference in peripheral blood T lymphocyte subsets between the two groups before treatment (P 0.05). After treatment, CD3,CD4 and CD8 increased (P 0.05) and CD4/CD8 decreased (P 0.05) in the two groups, compared with fluoxetine group. There was no significant difference in the (HAMD) score of Hamilton depression scale between the two groups before treatment, but there was significant difference between the two groups at all time points after treatment, which was significantly different from that before treatment (P 0.05). The HAMD scores of escitalopram group were lower than those of fluoxetine group at the 1st, 2nd and 4th week, and there was significant difference between the two groups (P 0.05). There was no significant difference in the curative effect and adverse reactions between the two groups (P 0.05). Conclusion: escitalopram and fluoxetine can enhance the immune function of elderly patients with depression by improving and regulating the function of T lymphocytes. Escitalopram can increase the immune function of elderly patients with depression than fluoxetine.
