赤峰地区幽门螺旋杆菌感染现状分析

【摘要】: 目的:调查来我院体检的健康人群及就诊的上消化道疾病患者幽门螺旋杆菌(Helicobacterpylori, Hp)感染情况,以分析赤峰地区Hp在不同年龄、性别、民族、生活地区以及不同上消化道疾病之间的感染是否存在差异。 方法:对1000例来我院体检的城乡健康人员进行14C尿素呼气试验,阳性者为Hp感染;对960名就诊的上消化道疾病患者经胃镜、胃粘膜活检组织诊断,行快速尿素酶检查及14C呼气试验检测,快速尿素酶试验和14C呼气试验均阳性者为Hp感染。 结果: 1.健康人群组Hp感染情况: (1)在1000例城乡健康人群组中,Hp阳性者644人,Hp感染率为64.4%。 (2)性别与Hp感染率1000名健康人群中,男性532人,阳性者344例,Hp感染率为64.7%;女性468人,Hp阳性者为300例,Hp感染率为64.1%,性别比较感染率无统计学差异(x2=0.003 p0.05)。 (3)年龄构成与Hp感染率1000例健康人群组中,年龄分布7-82岁每十岁分为一组,各年龄组Hp感染有所不同,各年龄组Hp感染率分别为33.3%、44.4%、61.8%、69%,70.3%、71.1%、68.6%、57.5%、53.8%,儿童Hp感染率为33.3%,随着年龄的增长呈上升趋势,51-60岁达高峰,Hp感染率为71.1%,60岁以后逐渐下降。 (4)民族构成与Hp感染率1000例健康人群中,汉族683人,Hp阳性者450例,Hp感染率为65.9%;蒙古族265人,Hp阳性者161人,Hp感染率为60.8%;满族39人,Hp阳性者25人,Hp感染率为64.1%;回族13人,Hp阳性者8人,Hp感染率为61.5%;经两两比较民族感染率差异无统计学意义(x2=0.141,0.052,0.143,0.144,0.181,0.028,p值均0.05)。 (5)生活地区与Hp感染率1000例健康人群组中城市居民734人,Hp阳性者457例,Hp感染率为62.1%,农村居民266人,Hp阳性者188例,Hp感染率为70.7%,农村居民感染率Hp感染率明显高于城市居民感染率,差异有统计学意义(x2=6.23 p0.05)。 2.不同上消化道疾病患者组Hp感染率不同上消化道疾病患者960例,其中慢性浅表性胃炎、慢性萎缩性胃炎、胃溃疡、十二指肠溃疡及胃癌患者Hp感染率分别为67.7%(302/446)、73.7%(143/194)、83.1%(108/130)、93.7%(133/142)、75%(36/48),十二指肠溃疡患者、胃溃疡患者Hp感染率高于其他上消化道疾病患者,差异有统计学意义(p0.05)。 3.健康人群组与上消化道疾病患者组Hp感染率比较健康人群组1000例受检人数中,阳性者644例,Hp感染率为64.4%;上消化道疾病组960例受检人数中,阳性者722例,Hp感染率为75.1%;上消化道疾病组Hp感染率明显高于健康人群组Hp感染率,差异有统计学意义(X=27.09,p0.05)。 结论: 1.在赤峰地区健康人群中Hp感染率为64.4%,高于全国平均自然感染率。 2.赤峰地区健康人群中,Hp感染率随着年龄的增长呈上升趋势,51-60岁达高峰,农村居民高于城市居民,但在性别、民族因素上无差异。 3.上消化道疾病患者Hp感染率高于健康人群Hp感染率。 4.不同上消化道疾病患者中Hp感染率不同,以十二指肠溃疡、胃溃疡患者Hp感染率最高。
[Abstract]:Objective : To investigate the prevalence of Helicobacter pylori ( Hp ) infection in the healthy population and the upper digestive tract diseases of our hospital in order to analyze the difference of Hp infection between different ages , sex , nationality , living area and different upper digestive tract diseases .
Methods : A total of 1000 healthy persons in urban and rural areas of our hospital were treated with 14C urea breath test , and the positive ones were Hp infection .
Hp infection was detected in 960 patients with upper gastrointestinal tract disease via endoscopy , gastric mucosa biopsy , rapid urease test and 14C breath test , rapid urease test and 14C breath test .
Results :
1 . Hp infection in healthy population group :
( 1 ) Among the 1000 healthy population groups , 644 were Hp positive , and the infection rate of Hp was 64.4 % .
( 2 ) Of the 1000 healthy people with HIV infection rate , 532 were male , 344 were positive , and the infection rate of Hp was 64.7 % .
Among the 468 women , the Hp positive rate was 300 cases , the infection rate of Hp was 64.1 % , and there was no statistical difference between the sex ratio infection rate ( x2 = 0.003 p . 05 ) .
( 3 ) Age distribution and Hp infection rate were among 1000 healthy population groups . Hp infection rate was 33.3 % , 44.4 % , 61.8 % , 69 % , 70.3 % , 71.1 % , 68.6 % , 57.5 % , 53.8 % , 68.6 % , 57.5 % , 53.8 % , 68.6 % , 57.5 % , 53.8 % , 68.6 % , 57.5 % , 53.8 % respectively .
( 4 ) The prevalence rate of Hp infection was 65.9 % in the Han nationality 683 , the Hp positive 450 cases and the Hp infection rate of 1000 healthy people .
The rate of Hp infection was 60.8 % among 265 Mongolian and 161 Hp - positive persons .
The infection rate of Hp was 64.1 % .
The infection rate of Hp was 61.5 % .
There was no significant difference between the two groups ( x2 = 0.141 , 0.052 , 0.143 , 0.144 , 0.181 , 0.028 , p < 0.05 ) .
( 5 ) There were 734 persons in the healthy population group living in the living area and the Hp infection rate , 457 cases of Hp positive persons , 62.1 % of Hp infection rate , 266 rural residents , 188 cases of Hp positive , 70.7 % of Hp infection rate , and the infection rate of rural residents was significantly higher than that of urban residents ( x2 = 6.23 p . 05 ) .
2 . Hp infection rate in patients with different upper digestive tract diseases was 67.7 % ( 302 / 446 ) , 73.7 % ( 143 / 194 ) , 83.1 % ( 108 / 130 ) , 93.7 % ( 133 / 142 ) , 75 % ( 36 / 48 ) , duodenal ulcer patients and gastric ulcer patients with Hp infection rate of 67.7 % ( 302 / 446 ) , 93.7 % ( 133 / 142 ) , 75 % ( 36 / 48 ) and duodenal ulcer respectively .
3 . In the healthy population group , the Hp infection rate in the healthy population group was compared with that of the healthy population group . 644 cases were positive , the infection rate of Hp was 64.4 % , and in the upper digestive tract disease group , there were 722 cases of positive persons and 75.1 % of Hp infection rate .
Hp infection rate in upper gastrointestinal tract group was significantly higher than that in healthy population group ( X = 27.09 , p < 0.05 ) .
Conclusion :
1 . The infection rate of Hp in the healthy population in Chifeng area was 64.4 % , which was higher than the national average infection rate .
2 . In the healthy population of Chifeng area , the infection rate of Hp is increasing with the age , 51 - 60 years old , rural residents are higher than urban residents , but there is no difference in sex and ethnicity .
3 . Hp infection rate in upper gastrointestinal disease patients is higher than that in healthy population .
4 . Hp infection rate was different in patients with different upper digestive tract diseases . Hp infection rate was the highest in patients with duodenal ulcer and gastric ulcer .
