沈阳地区腹泻患者和吸毒人群隐孢子虫感染的调查

【摘要】:目的 隐孢子虫(Cryptosporidium,CPS)是一种世界分布的人畜共患肠道寄生原虫,是引起腹泻的重要病原体。人体隐孢子虫病(Cryptosporidiosis,Cs)的绝大多数临床病例是由微小隐孢子虫(Cryptosporidium parvum,C.p.)引起。国内1987年韩范等在南京首次报道Cs后,全国十余个省陆续有病例报道。辽宁省至今尚未见隐孢子虫感染的病例报道及流行病学资料。由于缺乏一种准确可靠的病原学诊断方法以及临床医师缺乏经验,经常造成Cs的误诊和漏诊,延误了治疗。因此,建立一种快速、准确、方便、有效的诊断方法对于Cs的早期诊断及治疗具有重要意义。本实验从临床应用角度出发,进行以下研究:(1)调查沈阳地区不同人群即吸毒人群和腹泻患者中人体隐孢子虫感染情况,比较不同性别、不同年龄及不同职业人群感染率的差异,并同国外及全国报道的感染率比较,为流行病学研究提供资料。(2)比较碘染色、改良抗酸染色法(MAFS)及氯唑黑染色法的有效性和实用性,创建二步联合染色法检查粪便中隐孢子虫卵囊(Cryptosporidium oocyst,CSO),为疾病诊断和流行病学调查提供方法依据。(3)采用饮水途径给予小鼠免疫抑制剂建立隐孢子虫感染小鼠模型的尝试,为进一步进行隐孢子虫的致病机理和药物筛选等方面的研究奠定基础。 材料与方法 1.小鼠隐孢子虫感染模型及阳性对照标本的建立 10只10日龄KM小鼠(中国医科大学实验动物中心提供),不分雌雄,体重平均7g。小鼠粪便抗酸染色结果与已知阳性标本(解放军军需大学张西臣教授提供)做对照,确定阳性结果后,收集该小鼠粪便,用作本研究的阳性参考标本。 2.人体粪便标本的收集
[Abstract]:Purpose Cryptosporidium Cryptosporidium (CPS) is a worldwide parasitic protozoa of zoonotic intestinal tract, which is an important pathogen of diarrhea. The vast majority of clinical cases of CryptosporidiosisCsin human Cryptosporidium parvumum (C.p.p) are reported by Cryptosporidium parvumum. Cause. After Han Fan et al reported Cs for the first time in Nanjing in 1987, more than ten provinces reported cases one after another. There is no case report and epidemiological data of Cryptosporidium infection in Liaoning Province. Due to the lack of an accurate and reliable etiological diagnosis method and the lack of experience of clinicians, the misdiagnosis and missed diagnosis of Cs often lead to delayed treatment. Therefore, it is important to establish a rapid, accurate, convenient and effective diagnostic method for the early diagnosis and treatment of Cs. From the point of clinical application, the following research was carried out: 1) to investigate the infection of Cryptosporidium in different groups of people in Shenyang, that is, drug users and patients with diarrhea, and to compare the difference of infection rate among different sex, age and occupation. Compared with the infection rate reported abroad and nationwide, the effectiveness and practicability of iodine staining, modified acid-fast staining and chlorazolium black staining were compared. To establish a two-step combined staining method for the detection of Cryptosporidium oocystystus CSOO in feces, and to provide a method basis for disease diagnosis and epidemiological investigation) to establish a mouse model of Cryptosporidium oocyststachys infection by drinking water route with immunosuppressive agents of Cryptosporidium oocystystus. To lay a foundation for further research on the pathogenic mechanism and drug screening of Cryptosporidium. Materials and methods 1. Model of Cryptosporidium infection in mice and Establishment of positive Control specimen Ten 10-day-old km mice (provided by Center of Experimental Animals, China Medical University) had an average weight of 7 g, regardless of male and female. The results of acid-fast staining in mouse feces were compared with those of known positive specimens (provided by Professor Zhang Xichen, military supplies University of the PLA). After the positive results were confirmed, the feces of the mice were collected and used as the positive reference specimens in this study. 2. Collection of Human fecal specimens
