新生儿脐血碱剩余正常参考值与地理环境

【摘要】: 对新生儿进行脐血血气分析在临床上可以用来确切地反应新生儿体内的缺氧程度和酸碱失衡的情况,其中pH值,氧分压,二氧化碳分压等指标国内外报道的很多,可是鲜有关于碱剩余(BE)指标的描述和报道。本论文的创新之处在于对新生儿脐静脉血碱剩余正常参考值、脐动脉血碱剩余正常参考值与地理因素定量化研究。经过检索发现,只有一篇论文报道了新生儿脐血血气分析与海拔高度的关系,但也只是定性描述,像本论文这样对新生儿脐静脉血碱剩余正常参考值、脐动脉血碱剩余正常参考值与地理因素进行专题化定量化研究,从整个地理环境着手,整体的研究中国这一大的区域,还未见有报道。 本文的医学研究指标就是选择新生儿的脐动脉血碱剩余(BE)和脐静脉血碱剩余(BE),这两项指标都是判断新生儿窒息中的重要指标,通过对碱剩余(BE)和其他血气分析指标相结合的办法对新生儿的缺氧状况和酸碱中毒情况来进行判断。探讨中国新生儿脐动脉血碱剩余和脐静脉血碱剩余两项指标与所选各种地理因素之间的关系,从而建立它们之间的回归模型。 通过大量检索文献,以及向有关单位求购的方法,收集了全中国各地区新生儿脐动脉血碱剩余以及脐静脉血碱剩余的正常参考值作为两项医学指标。并根据有关地理著作和相关文献,选取了七项地理因素指标:海拔高度,年平均气温,年降水量,年日照时数,一月平均气温,七月平均气温,年平均相对湿度。将两项医学指标分别与七项地理因素进行了直线回归分析,逐步回归分析,多元线性回归分析以及非线性回归分析,这样得到了一系列回归方程,依据最优化原理,从得出的回归方程中选出脐动脉血碱剩余,脐静脉血碱剩余与各项地理因素之间的最优回归模型,利用得出的最优回归模型,选择有代表性的地点,将利用模型估测出的预测值和从当地搜集到的实测值进行比较,利用单因素方差检验法对实测值与模型预测值的可信性进行验证。最终发现实测值和模型预测值的一致性很好,可以利用该模型对各地的新生儿脐静脉血碱剩余正常参考值、脐动脉血碱剩余正常参考值进行预测。 运用直线回归分析,逐步回归分析,多元线性回归分析以及非线性回归分析四种分析方法得出新生儿脐静脉血碱剩余正常参考值、脐动脉血碱剩余正常参考值分别与各项地理因素之间的最优回归模型分别为:(?)_v=-2.034-0.285X_2+0.00814X_3-0.00120X_4+0.127X_5+0.156X_6±0.358F=79.420 P=0.000(?)_v的预测区间为[(?)_v-0.358,,(?)_v+0.358](?)_a=1.769-0.00296X_4±0.353F=1222.741,P=0.000(?)_A的预测区间为[(?)_a-0.353,(?)_a+0.353] 选取有代表性的地点进行估算和验证,然后将所估算的预测值与当地的实测值进行比较,通过单因素方差法对预测值和实测值进行检验,发现利用模型估算出的预测值和实测值一致性很好,无显著性差异(P>0.05)。
[Abstract]:The analysis of umbilical blood gas analysis for newborns can be used to accurately reflect the condition of hypoxia and acid-base imbalance in the newborn, including pH value, oxygen partial pressure, carbon dioxide partial pressure and other indicators at home and abroad, but there are few descriptions and reports about the index of alkali residue (BE). The innovation of this paper lies in the new students. The residual normal reference value of umbilical vein blood base, the residual normal reference value of umbilical artery blood base and the quantitative study of geographical factors. After retrieval, only one paper reported the relationship between umbilical blood gas analysis and altitude, but it was also a qualitative description, like this paper, the normal reference value of the umbilical vein blood base in the newborn. The residual normal reference value of umbilical artery blood alkali and geographical factors are quantified and quantified. From the whole geographical environment, the overall study of this large area in China has not been reported.
The medical research index of this article is to select the umbilical artery blood alkali residue (BE) and the umbilical vein blood alkali residue (BE). These two indexes are all important indexes in judging the neonatal asphyxia. By combining the alkali remainder (BE) and other blood gas analysis indexes, the anoxia condition and acid-base intoxication of new born children are judged. To explore the relationship between the remaining two indexes of umbilical artery blood base and umbilical vein blood base and various geographical factors in Chinese neonates, and to establish a regression model between them.
Through a large number of retrieving literature and the method of seeking for the relevant units, the two medical indexes were collected from the umbilical artery blood alkali residue and the normal reference value of the umbilical vein blood base in all regions of China. According to the relevant geographical works and related literature, seven geographical factors were selected: altitude and annual average temperature, Annual precipitation, annual sunshine hours, average temperature of one month, average temperature in July and annual average relative humidity, two medical indexes and seven geographical factors were analyzed by linear regression analysis, stepwise regression analysis, multiple linear regression analysis and nonlinear regression analysis, which got a series of regression equations, according to the optimization principle, In the regression equation, the optimal regression model between umbilical artery blood alkali residue, umbilical vein blood alkali residue and various geographical factors is selected, and the optimal regression model is used to select the representative location, and the predicted values from the model are compared with the actual measured values collected locally, and the single factor variance test method is used. The dependability of the measured values and the predicted values of the model was verified. Finally, it was found that the consistency of the measured values and the predicted values of the model was very good. The model could be used to predict the residual normal reference value of umbilical vein blood base and the residual normal reference value of umbilical artery blood base.
By using linear regression analysis, stepwise regression analysis, multiple linear regression analysis and nonlinear regression analysis, four methods were used to obtain the residual normal reference value of neonatal umbilical vein blood base. The estimated values are compared with the local measured values, and the predicted values and the measured values are tested by the single factor variance method. It is found that the predicted values of the model are in good agreement with the measured values, and there is no significant difference (P > 0.05).
