诺瓦克样病毒的分子流行病学研究

【摘要】:研究目的: 1.研究广州、江门地区5岁以下婴幼儿腹泻诺瓦克样病毒(NLVs)的感染情况、流行优势株及分布特征。 2.初步研究成人腹泻NLVs的感染情况及病毒基因型别。 3.探讨NLVs感染腹泻的临床特征及危险因素。 方法: 1.用酶联免疫吸附试验(ELISA)检测轮状病毒,对轮状病毒阴性标本用逆转录聚合酶链反应(RT-PCR)方法检测NLVs,并对部分NLVs阳性标本PCR产物进行纯化、测序,结合参考株相应的核苷酸序列进行进化分析,判断NLVs的流行优势株。 2.对NLVs感染腹泻临床特征的探讨采用与轮状病毒感染腹泻进行频数匹配的两样本比较研究;危险因素的研究采用1:1配对的病例对照研究。数据管理采用Epidata2.1a进行双份录入,SPSS12.0进行统计分析,,定性资料采用X~2检验,定量资料采用t检验或非参数检验。NLVs感染腹泻发病的危险因素采用SAS8.2进行单因素和多因素1:1条件logistic回归分析。 结果: 1.广州、江门两地NLVs的检出率分别为23.49%(70/298)和17.36%(25/144),主要感染2岁以下的婴幼儿,季节中的各个月份分布相当。33株测序病毒30株属于GⅡ-4型,3株属于GⅡ-3型。35例成人腹泻标本检出4株NLVs,测序的2株分别属于GⅡ-3和GⅡ-4型。
[Abstract]:Objective: 1. To study the prevalence, prevalence and distribution of Norwalk like virus (NLVs) in infants under 5 years old in Guangzhou and Jiangmen area. 2. To study the infection of NLVs and the type of virus gene in adult diarrhea. 3. To investigate the clinical characteristics and risk factors of NLVs infection diarrhea. Methods: 1. Rotavirus was detected by enzyme linked immunosorbent assay (ELISA) and NLVs, was detected by reverse transcriptase polymerase chain reaction (RT-PCR) in rotavirus negative specimens. The PCR products of some NLVs positive specimens were purified, sequenced, and the corresponding nucleotide sequences of the reference strains were analyzed to determine the prevailing dominant strains of NLVs. 2. To investigate the clinical characteristics of diarrhea infected with NLVs, two samples matched with rotavirus infection diarrhea were used to compare the clinical characteristics and the risk factors were studied by a 1:1 matched case-control study. In data management, Epidata2.1a is used for double input, SPSS12.0 for statistical analysis, and qualitative data for X2 test. The risk factors of NLVs infection diarrhea were analyzed by SAS8.2 with single factor and multivariate 1:1 conditional logistic regression analysis. Results: 1. The positive rate of NLVs in Guangzhou and Jiangmen was 23.49% (70 / 298) and 17.36% (25 / 144) respectively. 33 strains of sequenced viruses belong to G 鈪
