【摘要】：目的:通过比较三种不同的麻醉方法用于老年人ERCP时的多项观察指标,以期为老年人行ERCP寻求一种较为理想的麻醉方法,使老年人在行ERCP时更为安全和舒适。方法:选择60例择期行ERCP的患者,ASAⅠ~Ⅱ级,随机分为三组:右美托咪定组(D组),丙泊酚组(P组),咪达唑仑组(M组),每组20例。三组患者入室后按组别分别给予不同的麻醉药物。监测并记录三组患者不同时间点的HR、MAP、SpO2以及血浆肾上腺素和去甲肾上腺素水平;观察并记录整个手术过程中不良反应的发生情况。结果:D组患者的血流动力学较P组和M组平稳;D组患者的血浆肾上腺素和去甲肾上腺素水平低于P组和M组;D组患者术中不良反应的发生率低于P组和M组。结论:右美托咪定复合芬太尼用于老年人ERCP的麻醉时,能有效地调控围术期的应激反应,使患者围术期的血流动力学更加稳定,从而降低了患者术中不良反应的发生率,是老年人行ERCP时的一种较为理想的麻醉方法。
[Abstract]:Objective: to compare three different anesthetic methods for ERCP in the elderly, in order to find an ideal anaesthesia method for the elderly with ERCP. Methods: 60 patients with ERCP were randomly divided into three groups: right metomidine group (group D). Propofol group (P group) and midazolam group (group M) were treated with different anesthetic drugs. HRMAP at different time points were monitored and recorded. SpO2 and plasma epinephrine and norepinephrine levels; Results the hemodynamics of group D was more stable than that of group P and M; The plasma levels of epinephrine and norepinephrine in group D were lower than those in group P and M; The incidence of adverse reactions in group D was lower than that in group P and group M. conclusion: dexmetomidine combined with fentanyl can effectively regulate perioperative stress reaction in elderly patients with ERCP anesthesia. The hemodynamics of patients during perioperative period is more stable and the incidence of adverse reactions during operation is reduced. It is an ideal anaesthesia method for elderly patients with ERCP.
