七氟醚后处理对体外循环犬肺保护作用及机制的实验研究

【摘要】:目的:通过研究七氟醚后处理对体外循环(CPB)犬肺的保护作用及肺组织中Bcl-2和Bax的变化情况,探讨七氟醚肺保护的可能机制。 方法:实验用杂种犬12只,随机分为两组(n=6),左肺缺血再灌注组(C组)、七氟醚后处理组(Y组)。两组动物均腹腔注射戊巴比妥钠(25mg·kg-1),麻醉后气管插管、固定,股动、静脉置管监测平均动脉压(MAP)、中心静脉压(CVP),开胸并建立犬CPB。C组犬左肺行缺血再灌注,Y组犬左肺行缺血再灌注后七氟醚处理。于CPB前(T1)、开放左肺动脉即刻(T2)及停机后2h(T3)取肺组织光镜下观察病理学变化,采用免疫组化技术检测Bcl-2和Bax表达情况。同时在各时间点取股动脉血,行动脉血气分析,根据血气分析结果计算呼吸指数(RI)和氧合指数(OI),动态肺顺应性(Cd)。 结果:(1)两组犬肺组织光镜下病理改变及病理评分:光镜学结果:CPB前,两组犬肺组织结构清晰,肺泡间隔无明显增宽,肺泡腔未见填充物。开放左肺动脉后两组犬肺组织结构均紊乱,大量炎性细胞浸润,偶尔有肺泡壁断裂。停机后2h, C组肺损伤最严重,肺组织结构紊乱,肺间隔明显增宽,,大量肺泡壁断裂,肺泡腔塌陷,同时有红细胞漏出;Y组肺组织结构紊乱,部分肺泡腔塌陷及肺泡壁断裂,肺泡腔见炎性细胞。病理切片评分:CPB前,两组犬肺组织病理评分无明显差异(P0.05)。组内比较:两组犬肺组织病理评分T2明显高于T(1P 0.05),T3明显高于T(2P 0.05)。组间比较:T1、T2两组犬评分无明显差异(P0.05),T3C组评分明显高于Y组(P0.05)。 (2)呼吸功能参数变化:组内比较:与T1比较,两组犬T2、 T3时点RI逐渐升高,而Cd和OI逐渐降低,差异有统计学意义(P 0.05);组间比较:两组犬T1、T2Cd、OI、RI均无明显差异(P0.05),Y组T3Cd和OI明显高于C组(P 0.05),而Y组RI明显低于C组(P 0.05)。 (3)两组犬肺组织Bcl-2和Bax的表达:组内比较,与T1比较,两组犬T2、T3肺组织Bcl-2和Bax的表达明显高于T1(P 0.05);组间比较,两组犬T1、T2同时点肺组织Bcl-2和Bax的表达无明显差异(P0.05),T3肺组织Bax的表达C组明显高于Y组(P 0.05),肺组织Bcl-2的表达C组明显低于Y组(P 0.05)。同时T1、T2肺组织Bcl-2/Bax比值的变化无明显差异(P0.05),T3肺组织Bcl-2/Bax比值Y组明显高于C组(P 0.05)。 结论:七氟醚后处理对犬CPB导致的肺损伤有一定的保护作用,其机制可能与促进犬肺组织抗凋亡蛋白Bcl-2和抑制促凋亡蛋白Bax的表达有关。
[Abstract]:Aim: to investigate the protective effect of sevoflurane on the lung of dogs with cardiopulmonary bypass (CPB) and the changes of Bcl-2 and Bax in lung tissue, and to explore the possible mechanism of sevoflurane lung protection. Methods: twelve mongrel dogs were randomly divided into two groups: group C (group C) and group C (group C). Rats in sevoflurane post-treatment group were given intraperitoneal injection of pentobarbital sodium 25 mg 路kg-1.The mean arterial pressure (MAPP) was monitored by tracheal intubation, fixation, femoral artery and venous catheterization after anesthesia. The left lung of dogs in CPB.C group was treated with sevoflurane after ischemia and reperfusion, and the left lung was treated with sevoflurane before CPB. The pathological changes of lung tissue were observed under light microscope. Immunohistochemical technique was used to detect the expression of Bcl-2 and Bax. At the same time, femoral arterial blood was collected at each time point and arterial blood gas was analyzed. Respiratory index (RI) and oxygenation index (Oi) were calculated according to the results of blood gas analysis. Results the pathological changes and pathological scores of lung tissue under light microscope in two groups: before CPB, the lung structure of the two groups was clear and the alveolar septum did not widen obviously. There was no filling in the alveolar cavity. After the left pulmonary artery was opened, the lung tissue structure of the two groups were all disordered, a large number of inflammatory cells infiltrated, and occasionally the alveolar wall rupture. 2 hours after stopping, the lung injury in group C was the most serious, and the lung tissue structure was disordered. The pulmonary septum was obviously widened, a large number of alveolar walls were ruptured, alveolar cavities collapsed, and red blood cells leaked out. In group Y, the lung tissue structure was disordered, part of alveolar cavity collapsed and alveolar wall was broken, inflammatory cells were seen in alveolar cavity, and pathological section score was before CPB. There was no significant difference in lung pathological score between the two groups (P 0.05). Comparison between the two groups: T2 was significantly higher than that of T1 (P 0.05). There was no significant difference in score between the two groups. The score of T3C group was significantly higher than that of Y group (P 0.05). (2) changes of respiratory function parameters: compared with T1, RI increased gradually at T _ 2 and T _ 3, while CD and OI decreased gradually in two groups (P < 0.05). Comparison between the two groups: there was no significant difference in RI between the two groups. The T3CD and OI of the two groups were significantly higher than those of group C (P 0.05). The RI of group Y was significantly lower than that of group C (P 0.05). (3) expression of Bcl-2 and Bax in lung tissue of two groups: compared with T1, T2 in both groups. The expression of Bcl-2 and Bax in T3 lung tissue was significantly higher than that in T 1 lung tissue (P 0.05). There was no significant difference in the expression of Bcl-2 and Bax between the two groups (P 0.05). The expression of Bax in group C was significantly higher than that in group Y (P 0.05), and the expression of Bcl-2 in group C was significantly lower than that in group Y (P 0.05). There was no significant difference in Bcl-2/Bax ratio between T 2 and T 2 lung tissues. The ratio of Bcl-2/Bax in T 2 lung tissue in group Y was significantly higher than that in group C (P 0.05). Conclusion: sevoflurane post-treatment has a protective effect on lung injury induced by CPB in dogs, and its mechanism may be related to promoting the expression of anti-apoptotic protein Bcl-2 and inhibiting the expression of pro-apoptotic protein Bax in canine lung tissue.
